I have breast  but my nipples and areolas have not changed.
When do these usually start changing?
Oh I have been on HRT for over 8 months.

I am 10+ years postop and about 12 years on estrogen. About a C cup if I were a 38, but the nipples and aureolae never grew at all.

I am 28 months into HRT, and my areolas have gotten bigger.  Nipples have not changed however.

I think it is one of those YMMV.

This really has the most to do with genetics and your individual response to hormones, adro-blockers etc. Everyone is different. Basically, I wouldn't expect miracles.

This is one of those things that just varies tremendously...

For me, the area under my areola started to develop first and gradually spread out. Now 2.5 years into HRT, I have A cups and very slightly hypoplastic development. Thankfully, I always wanted "puffies" ;-)

  This may sound strange but you might consider piercing your nipples.... Any piercer will tell you  that piercings will enlarge your nipples, especially with a larger gauge. I pierced mine (now 12g) way before HRT and they got bigger then.

Yeah, results will vary. I've been on HRT about 8 months too, and my nipples have tripled in size (they were very little to start with though), and the aureole have gotten slightly bigger and darker. They changed as my breasts grew.

I was considering piercings as I always had inverted nipples (plus I like the look of nipple piercings), but they seem to have fixed themsevles.

lol sure.  ;D

Mileage on this varies but in general the younger you begin HRT the more likely the breast development will follow the Tanner stages.

6 years on HRT and implants and my areolas have grown and darkened but my nipples have stayed male small.
I have friends who had various AIS, Klinefelters and have all transitioned to female and some of them have the very large female nipples and areolas.
which seems to indicate that getting large areolas and nipples come from genitics rather than HRT?

Okay! Genetics mostly.
Well I need to email my to sisters and call mom and ask them what size they have.
I don't think so.
So I will just wait and see.

Oh look one of my nipple seem to be elongating.
I will let you know what happens.

Yeah I've been on HRT almost 9 months now and my nipples are way bigger and they tighten up when they get hard. ;) Now if the rest of my boobs would grow.....barely an A-cup , not filling it out yet

All I can tell you is hang in there.Most of my breast development didnt happen till around 15 months on HRT.Although YMMV.

Really? Mine were puffing up after 2 weeks now @7months there very large and puffy about x3 what they used to be and stick out like bottletops. I can't even remember what my nipples used to look like as a guy I wish I had some more boob mass to balance out there crazy size In warm weather I do look at them and think they look abit silly.

Pebbles like I said earlier YMMV.Plus mine are kind of small to begin with.     A part of me wants to get implants, then another part of me says, I had enough done on my body already.

Up Date
I'm in my into my 9th month of HRT now.
And things, nipples and areolas are growing.
The breast are sore and hard under the nipples.
This has been a constant for about the last 4 months.
Sometimes the breast, nipples area especially, get real sore for half a day or so and then ease up.

Now the right side has a bigger nipple about twice the size it stared out. 
And when I wake up in the morning after not wearing a bra the breast are conical shape.
Nipple and areolas really stand out forming  another mound on top of the breast.
And the left areola is now bigger than the right.
This is weird but still wonderful and exciting.

The areolas are darkening a little and now they shine in the light.
Nipples are slowly growing from the bottom up.
Still lopsided.  :D
The skin on the breast have gotten noticeable softer.
And the breast themselves are getting more girth.
I wonder what is next?
This is an exciting time.
I never imagined that that this could all really happen.
It just seem like a pipe dream 9 months ago.
