Survey is very wrong

- There are issues with two questions -- What is your gender with choices of males, female, transgender and prefer not to disclose? and How do you identify your sexual orientation? -- intended for Grade 7 to 12 students. A student in Grade 7 may not even know or understand the question, other students may use it as an opportunity to play a prank (and select an answer which is not true) and, it is no one's business.
