A question from a pre op..who hopes to be post op one day..how do you tell if you need a revision or cosmetic touch up on your SRS? Or are you all happy with the first result? Are some surgeons better and do not need the further surgery?

In theory most modern surgeons aim to do an all in one go procedure, which will not need further work.
Two riders to that.
Some of us who were early pioneers from the 70's and early 80's as I was, do find that as we get older we wish to take advantage of the improvements that have taken place. I had a revision last year which I remain delighted with.
Additionally people are not robots, thus the same technique applied to several different people will produce several different result ranging from superb to terrible. Thus some people even today find that they need a revision because something did not turn out as expected.
Finally, most of the well known surgeons are pretty much on a par. They all have their supporters and detractors. They all have aspects that they do well and others which they are not so good at. The important thing is to be realistic, to book a reputable clinic, and not some cut price back street deal.

Prior to the surgery, you'll have the opportunity to meet with a surgeon, and you can discuss your expectations and priorities at that time, as well as having a chance to see if there's a rapport. The vast majority of clients are satisfied with the outcome, and most clients take their time, and choose the surgeon that's right. I chose my surgeon based upon location, cost, experience, and typical outcome. I had seen photos of his work and was impressed. I was impressed with his aftercare provisions. Internet searches led to many satisfied clients, so it was a complete package. There were several other SRS surgeons I could have chosen I would have expected a quality outcome from, but I made my choice upon my specific needs.
Cost is only one factor, and we only have one chance to get this right. Take your time, ask questions and shop around, and you should do well. Keep in mind, it's a complex surgical procedure, and as with any surgery, there aren't any real guarantees. Still, an experienced and reliable surgeon can tell you what you can reasonably expect. After the surgery, proper dilation and after care is essential.

I had surgery in 1974 with Dr. Biber. Techniques in those days were not nearly as refined as they are today and I had the complication of having very little to work with. Dr. Biber promised to 'do the best he could to ensure I had an enjoyable sex life' and he achieved that goal but due to lack of available material, the cosmetic result was not as good - adequate but not average in appearance. I considered a revision a few years ago but surgery would risk loosing some sensitivity and I decided I am not about to risk "highly orgasmic" for appearance! Not only do I get to enjoy my orgasms but so does my partner! ;)

When I had GRS with Dr Bowers, she said I could come back in 6 months if I didn't like the results. At 8 months I began thinking that I might have to go back for a touch-up. I thought I was fully healed by then, but at 10 months I am far more comfortable with it, everything works as it should, and as a girl-friend said: It looks real. (I told her it was real, just not factory-installed.)
If you think you might need a revision, I would advise waiting at least a year or two. It's major surgery and takes a while to heal completely.
