Title: Filing name change in Chicago, Illinois.

So I'm getting all my ducks in a row here, was hoping I'd have my name changed by my birthday (June 10th) but the 7-week waiting period makes it seem unlikely.  Just want to make sure I have everything I need before I go down to the State of Illinois building on Wednesday.  So if anyone who has gone through the name change process in Illinois could make comments, I'd appreciate it.
-I will be filing a financial waiver affidavit since I am poor as hell. Do I need to bring proof of my earnings or anything?
-How much does it cost to post a notice in the newspaper?  Do I have multiple options and if so are some cheaper than others?
-I am using http://www.law.siu.edu/selfhelp/info/court/namchsup.pdf (http://www.law.siu.edu/selfhelp/info/court/namchsup.pdf) this to help me file, how does it look?  Useful?
