How many different papers do you have to fill out get get your name change? I looked at a few different websites and there all telling me different things. Do I only need to fill out Name Change Petition and Name Change Order? Are there any more forms I need to fill out before I file with the County Clerk's Office? I have no money for a lawyer.... Would I need one?
Call the clerk's office and ask. They will know.
That's really going to depend on what state you're filing in. When I did mine, I had five different things plus the attachment showing that I'd published it in the newspaper. Some states have different forms/a different number of forms though. I guess what I'm saying is, if you'd feel comfortable sharing what state you are filing in, you'd probably get more specific responses. Alternatively, do what Andy suggests, and just call the clerk's office. Fair warning though, they may be completely incompetent; the clerks in my county certainly were. :I
It's also dependent on your county and even city. I know my city specifically does not require one of the documents.

I emailed my county clerk and they told me to do the research myself. I did the research already but different websites are telling me different things for the same state. This is hard for me... Every state is different, but all I did was google "name change Wisconsin" and first hit was the instruction of the Wisconsin Court Systems website.
What state are you in?
You can go to the county clerks office and request the forms in person, they are required to provide copies.

I'm in ny state.
This what I got from one ny website: http://www.nycourts.gov/forms/namechange.shtml (http://www.nycourts.gov/forms/namechange.shtml) and this website doesn't mention needing these forms:
Request for Judicial Intervention (RJI) (original plus 2 copies) and an Index Number Application

To change your name for any other reason
You have to ask the New York State Supreme Court to change your name. To do that, you must fill out and file these court forms:
· Name Change Petition
· Name Change Order

Thanks! :) Yeah, nyc looks like it would be easier.
BTW: I done my Name Change (2 min), Divorce (4 min)& USA Naturalization (10 min) all in the same week. The process was so simple, but yes I was screwed like a little girl :'(
