I noted from one of your posts that you said you lived in California, and that English may be a second language for you. I grew up in Northern California. Some of the advantages of where you live: a) an entrepreneurial culture - people start businesses and encourage other people starting out. b) relatively high wages - this helps! c) a lot of diversity d) good community colleges and training programs. Talk to people and take advantage of everything you can.
Some steps:
1. One of the best things you can do is open a savings account and start regularly putting in whatever money you can (no matter how small). Begin!
2. Start taking steps to increase your long term ability to make money:
a) finish school. Get your high school diploma, or at least your G.E.D.
b) improve language skills (spoken and written)
c) go to college or enter an apprenticeship or both
d) build work experience
e) believe in yourself
f) build social skills
g) work hard, keep your eyes and ears open, always take on new challenges.
3. Learn to live inexpensively; everything you save, put aside for your future.
4. Go to work for a company that covers gender surgery (or start your own company).
5. Be very careful with borrowing and debt; with a good credit rating and a good job you may be able to borrow a significant portion of your surgery costs.
6. Take advantage of any retirement and savings benefits where you work, and use all company matching if you can. Later you may be able to borrow against your 401k, for example.
7. Proceed to become a citizen of the US as soon as you can, if you aren't already one. Be aware that transgender is a reason for political asylum, if you are from a country that persecutes transgender people.

Buy, rent a multi-room house. Put ads around for roomates. Rent out all the rooms in the house in such a way that not only do you cover your housing expenses but that you make some profit.
If you buy make improvements on the house as you can while keeping it rented out and then eventually you can sell or refinance it for the surgery money you need.
I bought a house for about thirty thousand dollars and over the years I refinanced it three times. I made improvements on it and rented it and now I have something I can sell for a small profit (small due to 3 refinances) or a place to live for free that gives me a small amount of income.
Oh btw because I own my own place I had a place to run my own business and be self-employed for several years.
