Title: Name change in NYS

Ok, I tried looking online for information, but it's mostly fluff and I am frankly uninterested in shuffling through garbage resources. If anyone has experience changing their name in NY or can direct me to a place with actual information, I'd really appreciate it. Thanks in advance.

I'm in the process of changing my last name, and it's far from hard.

Ok, so you fill out that form and then what do you do?

pretty sure you have to submit it and then you will eventually be in front of a judge who will swear in your new name. 

You might get unlucky, and the judge might want to see proof.  So beforehand if you have a therapist try to bring in a letter.

This is as much as I know, but I have not officially changed my NYS name.
