You very well could start before 20, you could even start before you're 19. I can try to assist with some searching, but it's really just scouring the web for information.
What part of Idaho are you from? That would be the first step in finding you some doctors. Also, how far are you able/willing to go?
Also, usually before starting T you have to go to therapy for 3-6 months.
I'd say your first step is to start seeing a gender friendly psychiatrist. That means that they must be a licensed doctor also capable of prescribing meds and writing legal documents for you. Start by calling your insurance company, if you have one. Get some names of just psychiatrist--don't mention gender to them. Then start calling some psychiatrist and ask them if they deal with gender issues. You should find someone eventually.
After a month or two of therapy, if you still feel this is what you want, start seeing an endocrinologist. First visit you'll have to explain to them what you are doing. They may be very unsupportive, in which case you'll have to find another one. Eventually, you will find one--either by luck or by research, your psychiatrist might even know of one. Your psychiatrist and doctor should have some communication together. Then after blood test and stuff, you can be prescribed hormones. You're pry looking at about 6 months to a year. I came out in March and didn't start hormones till August. I moved incredibly fast. Everyone is different, you may want to move slower. But that was the basic process I went through to start hormones.
It is a very emotional and eye opening process. I encourage you to enjoy it while it is happening, the ups and the downs. Make sure you have ample support from friends and your God, if you are religious. Doing this alone thinking it will solve all your problems is a mistake. You will still be the same person. You will still have the same problems, maybe even more.
