Title: Anyone has a sample recommendation letter for HRT

I would like my therapist to write one but she asked me to get a sample and I couldn't really find anything online.

The endo has asked for it but he doesn't have a standard one either...

Go to  http://www.icath.org  for an informed consent letter,  On the left click 'How does it work, then click link on #3.  This is the one I used.  Dani

Your therapist doesn't know how to write a recommendation letter? I would be finding a new therapist.
