I mean are they made up of the EXACT makeup and tissue and structures as a real female breast?
Reason why I ask, is because I HOPE to be getting surgery to have an implant inserted into my right breast, no not silicone implants but a medical device and I don' want any one to know that I am on HRT. Of course I would stop my HRT prior to the surgery.
Would a surgeon be able to notice that I have been on HRT once they cut into my breast by looking at the insides?

Are Male breasts and Female Breasts the Same AFTER HRT? I mean in insides, the tissues and all that stuff under the skin.

No, they won't be able to tell.
However, having surgery without your surgeon being aware you are taking hormones creates some issues, regardless of whether you stop pre-surgery - like no comeback for you if it goes wrong. Your medical types should always be informed, besides, they are legally and ethically required to keep their mouths shut..
Before puberty the are the same :) Then the hormones do there work,so no the are not the same.
Why stop them ? inform them what your using, as with all medication.
I am a NON-Transitioner. Don't intend to and certainly have no plans to in the future.
My sleep issues are MUCH MUCH MORE important in my life. As it is my ultimate goal. If if mean stopping my HRT for a long period of time so be it.
My sleep issues are so severe that I WANT TO LIVE. I will stop my HRT to make sure I get this device.
I don't want any external problems...
Whether you are transitioning or not, if you are taking hormones, your surgeon should know.
Let me put it another way..
Here in Aus, in the circumstances you describe, if you were to die on the table as a result of the surgeon being incompetent and the fact you were using hormones and didn't disclose it would mean the surgeon could simply walk away with no penalties. And it would come out during an autopsy and Coroners Inquest.

I agree with all your comments. And you are all correct. My life depends on this surgery, IF I GET IT. I am not sure at all if I will be getting the surgery and WHEN for the matter?
Would you recommend I stop NOW in hopes of surgery say 6 months from now. My life is my ultimate goal here. If I get this surgery it basically save me.
So far I have been lucky and have nothing has happened to me, but these things can:
High blood pressure -> I have
Stroke
Heart failure, irregular heart beats, and heart attacks
Diabetes
Depression
Headaches -> I have
So if it means stopping my hormones, then so be it. I am afraid if the find out that I am taking them, they might say. NO!!!

I would rather stop it all NOW in hopes of getting the surgery then have them say no for ANY reason.
I will find out in a month, if I get a letter of recommendation from my doctor. If so, then I can apply for my health insurance to cover the cost of the surgery.

As far as I understand most Dr.s will have blood tests taken before most types of surgery so there is a good chance they will notice
It is best to inform your Dr. of all med.s you are taking, most will require you to go off hormones and possibly other med.s for at least 1-2 weeks before the surgery but it is best to find out what your Dr.'s requirements are
Not sure what you mean by a medical device, but I would talk to your surgeon. If they are going to do the best surgery to protect and save your life I'd be very open with them.
Being on HRT is not a contraindication to surgery, it just has to be managed, you don't want to have a stroke or bleed out because you didn't inform them of info they need to protect you.

Usually your E level is monitored and when it is low they operate. Depends on the route of E, if pills or patches it is easy to stop, if on implants they need to do levels and calculate the best time.
To be honest it isn't a big deal, there are all sorts of medications that are monitored before surgery not just HRT. You will also have a blood test to check your platelet levels and RBC levels in case you may be a bleeder and they need to keep a transfusion for you.
Any surgery is a risk, give your medics all and any info so they can look after you, they will not make any judgement on you, your life style or anything like that. They just don't want to lose you!

Based on this and below comments you made, I feel you haven't really heard us or don't want to. *Points to my sig* You say you want to live, you say you need this thing to happen in order to live yet you're letting your fears and worries get the best of you. (We all do this in some area/at some point though) Look. The fact here is, you have to tell them you've been on E, blockers or any other types of meds you've been taking or have recently stopped taking and as for E and blockers, that is something you'll have to tell them no matter how much time has passed, especially if you plan to go back on it in the future. You want to live right? Then don't do something that could put your life in danger just because you don't want them to know. As someone else said, they
HAVE to keep their mouths shut anyway. So quit worrying about it and do the right thing by telling your doctor'(s) involved. This may not make you feel at ease but, that'll come once you over come these fears that are controlling your life and also putting your life in danger too. Before HRT, male breasts are the same as a cis girl's pre-puberty. After HRT, "male" breasts will be identical to a cis woman's post-puberty.
Your title says pre-HRT, but your post says you're on HRT; the answer is that yes, developing breasts will have more glandular tissue and breast structures than a man's and the person doing the surgery may well be surprised. However, cis men do get gynecomastia, which is breast development that would be typical on a woman - it's only abnormal because it's on a man [who doesn't want it]. The surgeon might assume that is the case for you. So they won't be able to tell you're *necessarily* on HRT, but they will know you have some real breast development.
Call me another vote for being honest with the doctor, because you can risk your life by denying some of the variables they need to know.
Everybody has nipples, I've always reminded myself. ;) Mine breasts look great to me.
