For those of you familiar with the Jim Collins Foundation:
I have done all the legwork for the applications to submit - except a letter from a medical professional or therapist. Today my doc said he'd gladly write me one but wanted to know what exactly he had to say. We got on the J. Collins website together and found limited info.
Anybody that has submitted or is submitting a letter, just what is my doctor supposed to write? I figured it would sound something like, Yes, this person is in fact who they say they are, and is well of health and mental being to proceed with surgery. He just wants a more specific guideline of what to include. And I am not sure what to tell him. The deadline is 11:59 July 31.

I'd recommend you do what my therapist did for my letter to my Endo.
Call the Foundation and ask them what they're looking for in a letter. No better source to go to than the people who need the info as to what info to give them!

Will do for sure. No contact number though, I've been looking. I emailed them through their "Contact Us" and checking it today see that it has bounced back and was not delivered.
I am running out of time, so any further help is really appreciated.
