So! I am having my FFS with Facial Team three weeks from Friday! :-X

I'm gonna use this thread as an open forum for thoughts and to document my surgery and recovery as it happens :-).

I'm super excited and also terribly nervous!

I'm doing forehead reconstruction, chin and jaw, and then rhinoplasty :-). And yes  I am staying at Jenny's

You will love staying with Jenny! Her new place is beautiful. Both Jenny and Nicola are wonderful, as are the FT folks and the hospital staff.

I was there for the first half of September. Surgery one month ago. Bruises are long gone, but I am still swollen and numb in places, as I was told I would be.

Paid my balance on the surgery last week, paid my balance on Jenny's Nest today, and I am all set! Now all I need to do is show up :-P. Wheeeeeeeeee! I'm nervous about the recovery but I'm really excited about the surgery itself. I don't look particularly masculine from the front, and generally I have no problems passing, but especially from the side, I have a prominent brow ridge and a very square jaw/chin. So while they don't seem to hinder my passability at the moment, I am quite insecure about those features and cannot WAIT for them to be gone :-D. I leave two weeks from Wednesday!

Hey all! Just a little update :-). I arrived in Marbella late yesterday afternoon. I was greeted by the lovely Nicola and spirited off to Jenny's Nest. The Nest is GORGEOUS (https://instagram.com/p/9Y7QjPGzLq/). Jenny is so sweet and accommodating, and the other girls here at the moment are awesome and a wonderful morale booster. I'm getting up to get ready to head off to HC Hospital for all of my preop tests and cat scans (I'll definitely post my cat scan photos when I have them), and to have my final consultation with the surgeons. Then surgery is tomorrow morning :-X. Anyway! I'll keep y'all updated as things progress :-).

Wow good luck! You look great of course but I understand. I have similar issues with the brow ridge. Would love to see your results and recovery, etc. Even after having transitioned 6 years ago, I sometimes wonder about doing the brow ridge and/or rhino. Mostly worried about recovery and how I could keep being 'stealth' while not letting on to anyone about it.

That depends on the surgeries. I was quoted for Forehead Reconstruction (with "Microcapillary Transplants" to fill in the edges of my hairline) and a Brow Lift.

Yeah I'm not sure what I'll need. Forehead reconstruction and chin at a minimum maybe.

Hey guys! A little update! So firstly, I cannot overstate how glad I am that I chose to stay at Jenny's Nest. Having everything organized for you in your preop and recovery is amazing, and the import emotional support you get from being around other girls who are either going through it themselves or have already gone through it cannot be overstated. I wasn't nervous at all before the surgery until the night before. The other girls who were already about a week postop had gone out after dinner for some drinks and I hung back to try to get some sleep the night before my surgery. I suddenly got very nervous and couldn't sleep and then got very emotional. I heard the other girls come back and I went downstairs and immediately started crying. Immediately I had three trans girls embracing me and uplifting me, and helping me through a difficult moment. It was trans sisterhood at its finest. After we all chatted for a bit I was able to get to sleep.

Jenny herself is just such a doll. She's visited me twice already in the hospital. And she always has a comforting word or touch and knows exactly what to say or do to make me feel better.

The surgery itself went very well. Dr. Simon, Capitan, and Bellinga said I had very little bleeding. The day of the surgery, once I woke up, I was a bit out of it and that day was pretty much a blur. Fran, the on staff nurse for Facial Team, is just a dream. He is so attentive to everything. Pain isn't really that bad. Mostly just discomfort and a feeling of pressure. I have lots of numb patches on my face, but they said that's normal. I snapped this photo yesterday when they briefly took all of my bandages off:

Already I am SO thrilled with my results. And I go, if I'm this thrilled now, then I can't wait to see more and more of it reveal itself as the swelling goes down more and more. More soon as I heal up more!

So, I'm now back at Jenny's Nest. Still no real pain. Jaw and chin swelling have REALLY set in while forehead swelling has actually subsided a little bit. I feel like there's a brick sitting inside my jaw and chin haha. The worst is actually all the drainage from my rhinoplasty draining down the back of my throat and not being able to blow my nose. They said that should get better over the next few days. They have someone coming at noon to do a lymphatic drainage massage, which I'm very much looking forward to. All of that said, it's still really just discomfort and a feeling of pressure rather than pain. And my forehead swelling has actually started to go down a little bit. I've also already regained some feeling in the center of my forehead. Here's an updated photo from the front, the profile, and 3 quarter view:

Somehow one of the things that scared me away from FFS was Lynn Conway's account of it and how it would be a gritty ordeal with constant pain during recovery. Now your experience seems to be the exact opposite which is good.

FT is coming for Philly trans health this year so I may book a consultation with them.

You're looking good, even so early in recovery and the result seems so natural. That's what I'd like to go for, maybe after 2 years on HRT (I reach one year Dec 1).

The experience could easily be an age related issue.  A little over 6 weeks ago I had full Type III FFS with a bunch of soft tissue work and I looked like I'd been hit by a train the first 3-4 days, but I also just turned 57.  I believe Lynn Conway was over 60 when she had Type III FFS.  FWIW, the pain really was not that bad, then again I may be blanking it all out.  ;-) .

True that. The pain and loss of feeling are my big worries.

Lynn also had hers in 1999 with Dr Ousterout. Back then things were somewhat different.

I think there are two major things at play. One for sure is age. Dr. Simon in fact said how he's always impressed 'how quickly the young girls bounce back.' My left eye was completely swollen shut when I woke up from my surgery. By the time I left the hospital 48 hours later, it had reduced to the same minimal swelling I had above my right eye (that said, I also wore the ice mask RELIGIOUSLY those first 48... I weirdly liked it... A lot... Dr. Simon had to tell me to stop wearing it haha). The second part is that Facial Team doesn't do much soft tissue work. They really only work on the bone. They'll do some soft tissue work if you really really want it (one of the other girls at Jenny's had a face lift in addition to her FFS, and one had a blepharoplasty, but this seems to be the exception not the rule). Other surgeons, like Zukowski (who I'm not knocking at all, he ended up in my top two choices and it came down to him or FT), does much more extensive soft tissue work as part of his standard procedure, which makes swelling and recovering from said swelling far less predictable.

That's interesting. Dr O I believe was the pioneer of the type 3 reconstruction. I guess I'll have to ask them (FT) about the pain when I see them next year.

I will say re: pain. I was told by EVERYONE who went to FT, it's not painful, just uncomfortable. I did NOT believe them. I thought that was just impossible. At least so far, turns out it is true. I've had zero pain thus far, just some generalized discomfort and numbness.

Yeah, I'm sure bone work only would make it "easier".  I'm older and had a lot of sagging face issues going on too.  Below is a list of the procedures I had done during an 8.5 hour surgery.

Type III forehead reduction ( coronal incision )
Outer orbital rim shaping
Reduce corrugator muscle
Eye brow lift
Rhinoplasty including reconstructive nasal valve
Upper lip lift
Jaw contouring
Chin osteotomy, reduce height add bone paste for additional 2mm projection
Upper Blepharoplasty
face lift
neck lift
lipo filling
    under eyes
    cheeks
    upper lips
    nasolabial folds

Right before I left I also had FUE hair transplants done too.  So I've put my face and body through a lot of stress.

Yeah that's A LOT of work at once. I did:

Forehead Reconstruction

Mandible and Chin Contouring

Chin shortening and sliding genioplasty (The genioplasty was a last minute tacked on procedure. After doing my cat scans the surgeons saw that most of my chin projection came from the lower part of my chin which was going to be removed. So they decided it would be best if they moved the projection of my chin a little forward at the same time as the shortening to make sure the forward projection was retained, but the result should be, and right now looks to be, a chin that's shorter but with the same amount of projection I had previously).

Rhinoplasty

So for me it was all bone work. I didn't touch my soft tissue.

We run a major site on fresh air and the smell of am oily computer.

Okay! So! Susan's went down briefly and lost some data, including my one week postop update. SO luckily I had typed it all out on my iPhone notes and then just copied and pasted into here, so I'm just reposting exactly what I posted before. :P

Here she is boys! Here she is girls! Here's my one week update :-).

So firstly, here's a picture:

The upper left hand photo was the day after my surgery. Then the photos  move in clockwise order finally ending at today which is the bottom left hand photo.

So, firstly, energy wise I feel pretty back to normal. I'm staying at Jenny's Nest (which is wonderful, and if you get FFS with FT I highly recommend staying here). Yesterday we went to the old city of Malaga. I was able to get through an hour long bus ride both ways, climbing through a twelfth century Moorish fort, walking through the old city, and climbing through a sixteenth century castle no problem. We were out for a total of 11 hours. Obviously I was tired at the end, but I don't think I was any more tired than I would've been preop after an 11 hour very active day.

In terms of pain, the first few days, my forehead, front part of my scalp, and my chin were COMPLETELY numb. So while I was on heavier doses of pain killers, I wasn't feeling much of anything anyway. I am slowly but surely regaining feeling in those areas. My forehead is almost completely back online now, and my scalp and chin have sensation, though it's still dulled. But every day is more and more.

I'm not so much in pain as I am in occasional discomfort. Especially when I near the end of a pain killer period. When I'm set to take my next dose within the next hour, I start to get kinda a dull generalized ache all over my face. It's really more of a nuisance than anything else and at this point I could probably just deal with it if I had to (which I do in two days when I'm supposed to stop taking pain killers). The weirder part is that as nerves come back online, you get random shooting pains in otherwise numb areas of your face, or sometimes extreme itchiness. I am told this is totally normal and will stop once I regain all my feeling, which they said takes 2 weeks to get most of it back and then you may have a few lingering numb spots for a few months post op, they said usually along incisions.

Normally, when they do hair transplants to feminize the hairline, they cut out a strip of flesh from where they made the forehead incision and harvest the hair from there. Since I didn't need to do hair transplants, my incision got sutured right back where the flesh was. So I actually already have all feeling back along the scalp incision already, cause I think my nerves just went back to their old place and then only took a few days to come back to life.

In terms of swelling and bruising, the swelling has gone down HUGELY over the week (as you can see in the pictures). My jaw and chin are still quite swollen, I can feel the pressure and pockets of fluid in it, but it's gone down enough where at this point I don't think a stranger would think anything of it other than I just have a wide jaw. I HIGHLY recommend if you're doing your nose and you start to bruise, invest in some thrombocid. It's a topical cream that's supposed to help break up and flush out bruises. It has worked wonders for me. At this point I have almost no bruising at all around my right eye, and only a small line at the bottom orbital ridge and right on the water line of my eyelid on my left eye. I was skeptical of using it, but it REALLY works. I apply it 2 or 3 times a day.

As part of the cost of surgery, you get two lymphatic drainage massages with their massage therapist Antonio... Who, if you like boys, is just oh so dreamy haha. I have such a crush on him... As do all the straight girls who come through here :-P. The massage was another thing I was slightly skeptical of, but I think it really really helped the swelling in the initial days after my surgery. I think the difference in the second picture (taken before my massage) vs the third picture (taken the day after) is a testament to that.

Generally, I have found FT to be a well oiled machine. Everything runs so smoothly. They send taxis to pick you up for every appointment you have at the hospital and then drop you back off again when you're done. The taxis are always impossibly on time haha. They also give you a phone when you arrive, so if any changes are made to any of your appointments, you'll immediately get a text to your phone with the time change (and they don't change any timing of appointments with less than 24 hours notice). Also, some surgeons can be cold and clinical. FT is the complete opposite. It feels like a warm family that you are being brought into. They are so wonderful at patient care and making sure you're comfortable both physically AND emotionally.

I'm looking forward to my postop appointment on Monday so that I can finally see what's under this nose cast! I didn't do much to my nose, just reduced the width a little, I had a SMALL bump which I had reduced (though not totally removed, because I *personally* think that perfectly straight noses look unnatural), and I had the tip of my nose turned up ever so slightly. From what I can see of the tip of my nose, it's exactly what I asked for and it looks perfect.

Generally, I'm ecstatic with my results, even through all the swelling. FT is known for doing naturalistic work (because they typically only work with bone). I'm only 27 (only a month ago), and one of my BIGGEST FFS fears was starting to look plastic surgery-y in my 20s. My results already look so natural and I'm thrilled.

Yeah... That's what I was going to say... If you're presenting male at work and you have FFS, then sure, people will notice. But if you're already presenting female, and you take a week or two off, it's highly unlikely. More likely is that people will notice you look 'different' but not be able to put a finger on why.

When my co-workers asked, which they should not because I was out on medical issue, I told people I had sinus surgery to deal with a congenital defect. 

I am 57, transitioned 34 years ago, stealth for all those years.  I am 8 weeks post-op this coming Wed, 5 weeks hair transplants along hairline.  I had very aggressive Type III work done.  I was away from work for 5 weeks.  When I returned I still had slight bruising and a fair amount of swelling, plus you could see the redness along my hairline where the transplants were placed.

I think we give the general cis population much more credit regarding their ability to recognize a trans person, especially one who they associate with a specific gender.  A few may think you had cosmetic surgery, but women do that more frequently anyway.

Thanks for that. All good info. Also, did you feel like as you got older, your brow bone showed more? I think I'm worried about that as well. (hence looking into FFS again). I transitioned 6 years ago myself.

Yep, my brow was definitely more noticeable, particularly as my face aged.  Age related issues, fat loss and skin sagging, made my brow more obvious.

I transitioned early enough where my skin was not damaged by years of androgen, but all the years out in the sun definitely aged my face.  I am of the belief that not everyone needs FFS, especially if they transitioned in their teens (after puberty) or twenties.  However you should consider saving for FFS at some point in time.  I had an 8.5 hour surgery and I'll admit the first 4 weeks or so were pretty tough on me.

Thanks! And yeah, tons. My jaw is actually wider now than it was preop. But as they showed me on the cat scan, my actually bone is MUCH narrower and the height of my jaw/chin has been greatly reduced. So all of the extra width comes from swelling. I can feel and see the swelling on the jaw/chin easily. They also told me the same thing about my nose, but I don't really feel swollen there.

Couple of questions, not too sure if anyone else asked this but could you break down the cost (including the cost of staying at Jenny's) and how long you spent in Spain for your recovery?  How was the flight back?

So far, my top choices have been either DiMaggio or Facial Team due to their excellent records from what I've read.  I'm planning on getting similar work done with my brow, nose, chin and jaw and wanted get some details.  How did you come to choose Facial Team over other doctors?

The cost of the surgery was €19,300. I don't remember the exact pricing quote but that was the total. It included the surgery, the anesthesia, two nights in the hospital, and two lymphatic drainage massages.

Jenny's was €2,680 when I stayed there and it was for 11 nights (two nights are spent in the hospital). My surgery was on a Friday so I was in the hospital for two nights after that, and then got back to Jenny's on a Sunday and spent 7 more nights there. I haven't headed home yet. I went to Granada and now I'm in Madrid :-). But I could have gone home on Monday.

I chose Facial Team cause I liked how naturalistic their results are. Their philosophy is that certain bony structures grow thanks to T during puberty. They are easily identifiable. All they do is remove those structures and reset your facial structure to what it would have been had it not known T. I'm only (freshly) 27, and one of my biggest fears was starting to look 'plastic' in my 20s. So that philosophy majorly appealed to me. Also I liked that thanks to techniques they've innovated, that in my case, there wasn't going to be a single visible incision or scar anywhere. My forehead incision was in my scalp and is covered by my hair, and then my jaw and chin incisions are inside me mouth. Then there was just comfort level. I consulted with a number of surgeons. It came down to FT and Zukowski for me (who I also felt very comfortable with). But I had prominent brow bossing and ultimately decided I needed a reconstruction not just bone burring.

Just to give you an idea, this is a profile before and after (I took the after profile pic two days ago, there's actually been more swelling and bruising reduction since then). But this shows you more clearly the brow work that was done and the nose/genioplasty work. The genioplasty was actually a last minute addition to my procedures. After looking at my cat scan, we saw that all of my chin projection came from the very bottom of the bone, which was going to be removed when they shortened and tapered my chin (I had a very long square chin). So to counteract that loss of projection, they did a genioplasty as well as the recontouring at no additional charge.

So the swelling continued to go down. My nerves are starting to wake back up, leading to weird itching/random shooting pains, tingling etc all over random parts of my head and face. The only places that really still have major numbness are my scalp, and the left side of my chin (the right side of my chin is totally back online). But they're slowly getting better. Each of those places continues to get more feeling day by day, sometimes hour by hour and sometimes very suddenly. For example, yesterday I was lying in bed. Until then my whole forehead except for the center of it had been totally numb. Suddenly I got all of these shooting pains and tingles in my forehead, and then after it subsided I felt it my forehead and the whole thing had feeling again! It was crazy.

FT's patient care continues to be amazing. Even though I'm no longer in Marbella, whenever I send emails to my patient coordinator, she responds within 24 hours, making me feel like I'm taken care of no matter what.

It was bruising not swelling. Very common with rhinoplasty even when it's done on its own and not part of FFS :-).

Overall, I feel great. There's still some stiffness in my jaw and chin, and still a bunch of swelling there, but it's not noticeable to anyone really but me. The numbness on my chin and forehead had totally dissipated. I have some odd feeling in my chin that's hard to describe, but it's no longer numb. Now the only place that is still numb at all is the top of my head right in front of the scalp incision. The sides of my head are totally back online though. Occasionally I get weird pains around the endotines that are holding my eyebrows up haha.

Psychologically, I feel lotsa feelings :-P. I've definitely experienced some postop depression. That said, my confidence in my passability is now through the roof. Two anecdotes: one is that I'm taking two acting classes with the same teacher. One is specifically for LGBT students, so they all know I'm trans. I started that class pre my FFS. Then one is an on camera class that's open to whoever that I started after my FFS. Before doing a scene in the latter class, the teacher always asks "how are you doing?" And he wants a real answer, not a generic "fine," "I'm doing well," etc. So I said "I've been feeling a bit emotionally raw the past several days." And he responded well, you've been going through a lot, are you comfortable sharing with the class? So I did. And everyone was SHOCKED that I'm trans.

Second story is I participate as a writer in a concert series where you get assigned a singer you've never met and you have to get to know them and then write a song for them. So I met my singer. And we were chatting for over an hour. And then it comes up that I'm trans, and she also FLIPPED HER ->-bleeped-<-. She couldn't believe it. And then when we left the bar she was staring at me and I go, "what?" And she goes... "I'm trying to see it and I just can't, you're just SUCH a woman." And then I also have been getting hit on like whoa haha.

So yeah! That's it!

Right now Facial Team is at the top of my list.  I may end up talking to Dr. Spiegel but I'm still doing research on him.
I've had some great results on HRT and can say I pass 98% of the time, but my brow is a Type III and I think I need someone that can give me a nice fem forehead.lol   Like yours! :)

I have another meeting with FT. 

Hopefully, I'll be less on the fence about my choice after a few chats with them.

At this point I only have a small amount of residual numbness in front of the incision on the scalp. But that's it. My jaw and chin seem to shrink every day still. I think it's done and then it goes down even more. FT said it'd take six months, so I probably shouldn't be surprised haha.

You're looking amazing Dana! The Facial Team 'natural' look has worked its charm again :-)

I'm at 5 months post-op today. I think where you at at now at 3 months is when you see the biggest changes. I'd imagine something like 90% of the swelling is gone by 3 months, so it's hard to keep track of that remaining 10% or so around the jaw and chin.

Last week though I was going through my old photos on Photobooth at work and I found an old photo from exactly a year before, so I made a quick comparison shot. I was shocked at the difference in my jaw and chin, as I am now so used to my newer looking face.

Thanks Charlotte! And yeah, I do the same. I've gotten really used to it and sometimes I forget what a difference there is, especially considering as you said, how natural the facial team look is. And then I put a postop photo next to a preop one and go, whoa! Haha. One of my favorite things also is how many of my friends and family say, 'The results are so natural, if I didn't know you had surgery I'd have no clue you had surgery.'

Hi, I have a question for you!  After your jaw and chin, did they give you a black elastic band that goes around your head that you are supposed to wear?  Is it necessary, and if so, when and how long did you use it for?  I find it very uncomfortable especially when sleeping..

Hey there! Yes I did, it's to make sure the muscles and soft tissue reattach to the bone in the correct place. And you wear it for 14 days if I remember correctly.

Did Facial Team suggest a face/neck lift? I had an in-person consultation with Dr. Simon and he made no mention of it, nor did it appear in my quote. 3 other surgeons thought that the amount of Chin & jaw reduction in my case, would lead to excess skin, and she suggested some level of neck/face lift.

Hi Taylor, I just had a consultation with Facial Team. Even though they agree I would benefit from a face and neck lift, they didn't suggest one to go with the bone work they recommended. They said waiting 6 months to a year to see how the skin settles, then doing a second procedure, is the best plan.

I've heard the same from other surgeons.

Anyway, Dr. Simon didn't bring it up at all until I asked. I got the impression he thinks the face lift can be done by anyone once the initial work is healed.
