Title: Bottom surgery without male hormones?

 Ah, hello! So, I'm Luna and I identify as Non-binary. Born as a female, I'm not fully comfortable with my *ahem* lower parts, and I'd like to get bottom surgery. Just to be clear, I do't want to be a man- I just don't really want the lower pieces I was born with.

As such, I wanted to know if it were possible to get them done without taking male hormones? I don't want to grow facial hair or anything like that, and I don't want top surgery, so I was just wondering if there were health concerns or maybe something else I could take?

Hi Luna and Welcome to Susan's,

I don't see ant medical reason that what stop you having surgery. Many cisfemales have hysterectomies etc. Not sure how you would go for  reconstructive penile surgery. If you decided to have your ovaries removed you would need replacement hormones.

Ah, thank you! All those links are very helpful- though I was thinking something more along the lines of FTM? It's a bit hard to really describe...

Hi there,

If you're talking about a hysterectomy, it is completely possible to have it done without anything else. The doctor performing it may require a referral from a mental health professional if you tell them it is because you are trans. If you don't tell them you are trans, they may feel iffy about it unless you are a certain age. Younger people (who don't identify as trans) often find it difficult to get surgeries that involve sterilization.

If you're talking about bottom surgery (phalloplasty or metoidioplasty), the answer is a little hazy. The WPATH guidelines that most surgeons claim to follow state:

- Persistent, documented gender dysphoria
- Capacity to make informed decision
- Age of majority (typically 18)
- Otherwise healthy
- 1 year on hormone therapy as appropriate to your gender goals (unless unable or unwilling to take hormones*)
- 1 year living in a role congruent with your gender identity

That starred section is the hazy part. While it says that verbatim in the guidelines, I have never heard of someone getting bottom surgery without hormone therapy. It is essential for metoidioplasty if that is what you are looking into.

There is an FTM board here with a bottom surgery section. We don't have a lot of active members who are pursuing bottom surgery at the moment, so your best bet to get the most up to date information is to contact individual surgeons that you'd be interested in going to. Only they would be able to tell you what they would accept in terms of meeting the requirements.

I can't say I know much about this, but for bottom surgery testosterone enlarges the clitoris before surgery. It might be quite important.

My endo was talking about the spectrum of treatments possible for people and she had one MTF patient who wanted bottom surgery without any HRT.  So, yes, I think if you can find the right doctor and have the right reasons, you could get FTM bottom surgery without the corresponding top changes.

Sometimes I wonder exactly what I should do to feel more comfortable in my body as an Agender person, it's nice that you know what you want.
