I hope this is not to out there but why they do not make hhhmmmm something like a butt plug thing after srs/grs ? Where you can just keep it up there and open? Is there a danger to that or like does a neovagina not have a grip (guess that is the right word ) like the anus.

The muscles don't exist at the mouth of the vagina to hold anything in. In addition, the mouth of the vagina needs to be dilated as it can close up. Sorry but you have to lay there with one hand holding everything in place if you want to do it right. If you use a harness, there is a possibility you will not supply sufficient pressure to maintain proper depth.
The explanation, according to Brassard's nurses, is twofold : risk of infection, and risk that the skin will grow *around* it while it's healing. (I got the impression that something like that might be safe a few months later, but by that point, why would I trade one 20-minute dilation a day for wearing something all the time? :) )
Dena, we have the same muscles cis women do (in fact, those PC muscles are what make dilation a pain early on). I can definitely both clamp down on the dilator and push it out with them. Of course, they're not the same as the anal muscles, in cis women or in us, and they *do* get tired.

A few surgeons use a vaginal stent post-op which is basically the same idea. Its not very common though.

It's for a lot longer than the first week. I think Andy Ives may use them, and there's a European surgeon too.
I'm in the begining of the 4th week of dilation and a butt plug type would not be doable as the end needs to be narrower and I've not seen a 7 inch butt plug.
The trick to hold the dilator in place is to lay a towel folded to a 1 ft width and long. Lay that under yourself, after inserting to the depth just pull the towel up to your chest and it will maintain pressure and you can do what you want .

Thanks Anony. Yes ...went out last night for dinner for the first time since surgery, almost 4 weeks ago ...still some minor swelling ...the FFS surgeon was absolutely brilliant but the whole process was amazing ... thanks to my surgeons at PAI.

I find that inserting my dilators fully, holding it in place briefly, then simply crossing my legs works. I am six months post-op and still have well over 6 inches of depth and can fully and easily accommodate Mr. Orange these days without a second thought.
