Welcome to Susan's Place. I was on Premarin for many years and results can vary depending on the person. First, it's not the drug but how your body responds to it. When you first start with estrogen, it can cause morning sickness symptoms. I had a few and the best solution to the problem is to take your pill with a meal or at least with a snack. I always eat breakfast so I take it with that meal and now I like to include some fat as estradiol (estrogen) is fat soluble and will help buffer it. My favorite fat is whole milk but others will work as well.  After your body adjust to estrogen, the nausea and mood changes should level out.

I would suggest that you ask for another source of estrogen. Premarin is made by separating hormones out of the urine of pregnant mares (female horses). Not only is the very idea kind of gross, but the mares are tied in a standing stall that is barely big enough for them to fit in for 11 months straight and then, when the foals are born, they are either dumped in a field to die of starvation or are sent to slaughter. Some are saved by rescue groups, but the majority are killed. The mare is then immediately rebred and put back in the stall for another 11 months. When the mares eventually stop becoming pregnant, they are slaughtered as well. It is a horrible product and its production incredibly cruel.

Aside from the animal cruelty aspect, premarin is also not very effective at inducing feminization, and contains estrogens that are unique to horses, for which the human body doesn't have the enzymes etc in place to deal with. IIRC, it nearly doubled mortality in the 2002 WHI study, and is the main reason doctors are now so reluctant to prescribe HRT to postmenopausal women. It's an old drug, and would have been abandoned long ago except for slick marketing by the pharmaceutical company that produces it. You're far better off with bioidentical estradiol, which is more effective for feminization and has a much better side effects profile (especially transdermal or injected).

Bioidentical etrogen is available nowadays without problems.

Its simply not copyrighted and thus not as widely marketed.

Its posible to use it sublingually which bypasses the liver with intake.
This way its supposed to be less straining on the liver, and less causing clotting effects.
Drawback is a faster entering into the blood stream so to avoid spikes and lows, which might affect mood, it might be preferable to spread the daily dose in a few small ones instad of for example two big ones.
Just talk it through with your doc.
