Your spouse should be told as soon as possible for reasons of trust. Family maybe depending on your relationship with them and the rest of the world when you feel like it. While it doesn't happen often, some people don't like HRT so there is no point in telling a casual acquaintance until you are a little farther along. Estrogen isn't as powerful as testosterone so you have more time to work with.
All my closest friends I told long ago. I told the world generally the same time I decided to start HRT for the reason that I needed a point in time to start counting RLE toward the possibility of SRS.
I came out before starting HRT. From an FTM point of view, testosterone acts very quickly and there's really no telling what changes you'll get or how fast you'll get them. I felt it would be best to tell people in advance so that they wouldn't be surprised or confused about any changes they noticed.
Besides my Susan's friends, there are 20 people who know. I told my son this morning. So everyone in my family knows, except my 85+ yo parents.
I started Spiro just over two months ago, and started estradiol TODAY!!!
