Title: Letters of medical necessity for FFS

I was wondering if anyone have successfully used medical insurance to pay for FFS. I live in California and I have blue shield. My plan covers TS related surgeries but they need doctor's letters. I don't know if the letters need to come from my primary physician (she prescribes my hormones) or a therapist.

I have Kaiser / Group Health and they will pay if it is a medical necessity. I'm not sure what that means without a recommendation. Because me saying it is a necessity is not going to cut it. I have been told I would need letters from both the doctor and the therapist.
