I don't have enough information to answer fully. However, according to anecdotes I believe there are ways to work with or around an orchi. Usually they discourage that to give the most tissue available for re structuring. I encourage any to correct me but I believe the penile inversion needs more penile tissue than scrotum. This is all based on me having read many posts and concluding that.
