His prices are extraordinarily reasonable, and he does have some experience doing FFS ... For brow shaving, hairline advancement, brow lift, upper lip lift, and jaw shaving, I was quoted $9000 (and that's with everything included, facility fees, anesthesia, etc.)

You might not get the results you want with simple brow shaving -- if that bone is very thin, there's not much that can come off. In which case, the Type 3 reconstruction would be indicated. You can only know what kind of bone is in there with an X-ray of your profile.
