Here's a question...at what point do natural breasts through HRT start "cupping" or getting that crease and hang?
About 4:00 if you don't wear a bra.
Seriously, mine have just started to after 2 2/3 years,but it varies. Genetics, dosages lots of things can affect it.

I'm sure it varies but I have been watching for it. I started seeing the "crease" about 3 1/2 - 4 months but only recently have I seen what I can call a "hang" and I'm at 6 months The smile on my face has gotten correspondingly bigger ever since I could see them as small bumps. (and they feel nicer all the time too)

It's genetics and muscles...
I had significant breast development 4 years ago and went full time three years ago. I work out, including lifting weights. They are still perky...

In addition, overall bodyfat level makes a difference. If you maintain an athletic bodyfat level by working out they may not ever sag.

9 months in and they are starting to bounce when I walk. Not much hang though.
For myself, within one year.
But this is so variable and more dependent on genetics than estrogen. What size are your female relatives? If they are an A or small B, you may never see any crease under the breast. If larger, then you will see full development in 3 to 5 years.
