I feel ashamed to admit it, but my orgasms have shrunk to the point of virtual non-existence. I'm 1.5 years on HRT, patches first then shots. I can still orgasm, but they are these tiny weak little orgasms that really don't feel like much like my old male ones. Nothing comes out anymore so I guess that means I'm 100% sterile now, but I didn't realize I'd dry up completely. This whole thing makes me question my transition to some extent. A major thing I had hoped to fix was having sex as the right gender. I didn't expect it to become what it has since I never heard anyone talk about this sort of thing. I'm starting to wonder why bother to orgasm, there's almost no need for it. I guess that makes me more like other women to some extent :) Has anyone else had this happen? Do I just need to learn how to include my mind more in sex perhaps?
I think you may be confusing orgasms with ejaculation. You may very well lose the ability to ejaculate. However to orgasm try experimenting other ways to make you orgasm, other erogenous zones, penetrative orgasms, toys. Think of orgasm I guess like a girl. You might find this new type is more powerful. It might take a while to retrain yourself to think and behave this way too, don't rush it, just let it happen.
I hope that helps. I personally don't miss my male orgasm/ejaculation at all.

Okay, I'm going to be blunt.
As an AFAB, I can assure you that NOT having an interest in sex and orgasms is a myth! Not only should you be able to orgasm, you might find that you can have more than one. Like it was mentioned before, "ejaculation doesn't mean no orgasms," it simply means retraining your body to have a different type of orgasm.
I agree with Bari Jo. You need to retrain your body and more importantly, your mind.

Although there is very little ejaculation, my own orgasms have improved remarkably. They are more full body - head to toe, the build up is better and they last longer. I will say that they are perhaps not as momentarily intense as a male orgasm but the whole experience is better and more fulfilling.
I'd definitely talk with your doctor too, and look at the different ways you are achieving them.
This will somewhat depend on your last blood test. Some testosterone is needed for sexuality and the blockers may be doing to good of job. If after discontinuing the blockers your levels are low, a patch could be used to raise them to feminine levels. Normal levels are 14-76 ng/dL which is well below the 300 minimum for male levels. I clock in at 36 which isn't bad for somebody post surgical.
I can confirm this as my T was 4-7ng/dL when I used Spiro and I had no desire to engage in sex or masturbation point blank. When they tweaked my medication and my T was in the range of 22-24ng/dL I become more sexually aroused at the thought of intercourse with someone even if I did not engage in it. That said before starting HRT I was the person wanting sex five times a day with my poor ex-girlfriend and still wanting more!!
The thing I noticed is the feeling of release is still their but less intense and almost unappealing compared to when I was still pre-hrt.

My T was < 3 forever, just recently at 8ng/dl. I have no problems whatsoever with gaining orgasms... pretty much ever. HRT only made it worse, progesterone in particular (worse as in my body demanding attention). Also post op since 2001. And as weird as it sounds I still have some kind of ejaculate that lubricates my vagina. Some day I'll figure out whats making it.
I think it sums up to different strokes for different folks.. some peoples bodies respond differently than others. Then again my body has always been an alien thing.
