Hi all, I've been practicing ketosis on and off with paleo for about six years. These years have seen my physical health improve with a reduction of mental stress( obviously not taking away Dysphoria). My question is has anyone experienced ketosis whilst under going hrt? I really don't know who to ask as endo knows nothing about ketosis and my primary doctor ( an expert on diet) said that his hormone knowledge is limited.

Yes. I have, along with extended intermittent fasting. It had no effect on hormone levels. In my case those levels are independent of diet.
If you are maintaining a low bodyfat level though that may have an effect on new fat being deposited.

Hi Deborah, I hoped that would be the case and makes sense that there probably shouldn't be any issues thanks for telling me of your experience and yes, not really for weight loss and am excited about getting a little fat in the right places.

I agree there should be no affect on your hormone therapy except that a true ketosis diet burns fat and you really need to grow some fat on your thighs and upper chest. Ketosis will not get you there and if your go into keto acidosis that can be dangerous. In the 1970's, a liquid protein diet was promoted as a quick weight loss supplement. A few people went into keto acidosis and died from the diet supplement.
In my experience a diet high in fresh vegetables and fruit will provide almost all the necessary vegetable fats to get you where I think you want to be.
Keto-acidosis is really only a risk for someone with diabetes. A keto diet, if done properly, is not high protein anyway. It's simply low carb, high fat, and adequate protein. It's also high in low calorie green leafy and cruciferous vegetables.

Hi Dani, thanks for that! I'm eating lots of salad and some low sugar fruits. Deborah is right about the keto acidosis, and the moderate protein , I think the danger zone is around 14mml blood ketone level whereas mine and any healthy person would struggle to get higher than about 5 even with extreme carb reduction ( I've managed no higher than 3.7mml). Some (though not all) doctors still get confused about that... which is a shame because it's certainly helped a few( myself inc) out of some potentially serious conditions.
I've been in a constant state of ketosis from day one of hrt. No adverse affects on hormone levels, feminization, fat redistribution or breast development.
I also have been intermittent fasting for over a year now on a daily basis.
From the research I could find of girls going through puberty with low BMIs it appears that if there is not a lot of fat for breast growth, the timeline just extends and simply takes longer rather than being stunted.
I also hypothesize that if you are combining keto with IF one may benefit from the increased levels of hgh, which of course are highest during puberty.
I am just over a year into full transition levels, with six months before that of low dose hrt, slowly titrated over time due to the slight risk of seizure activity, also the reason I'm on the keto diet.
Needless to say fat redistribution is going well, breast growth is coming along, close to a full 34 b cup now- yay! That's with about 115 lbs of weight loss during that period. So obviously weight loss + keto did not impair breast growth for me.

When I take spironolactone, It is recommended to take it with food. I was wondering when you fast have you seen any issue in the effectiveness of Spironolactone when not taken with food? (Sorry, I'm assuming you take Spironolactone)

I Intermittent fast every day and am on spiro too. I take my morning dose without food, usually at least eight hours after my last food and eight to ten hours before I eat again. That has not made it less effective. My T is still undetectable. It also hasn't led to any other stomach problems.

No issues with efficacy here either. I take spiroenlactone in am, always on an empty stomach. Total T is under 10.
I do find that with keto and IF, combined with spiro that I need to constantly replace electrolytes, especially salt and even potassium, which o note since it's recommended to avoid foods with excess potassium while on spiro. Seems like having no water weight reduces electrolyte storage/maintenance.

I have the same experience with electrolyte replacement and have a fairly high sodium intake now. I don't pay much attention to potassium and get a fair amount of that from potatoes and other foods; my blood potassium level remains normal. I did have to start supplementing magnesium too but that was even before starting this eating method.
