Hi, thanks for approving me into this group. I'm a transgender from malaysia but has stop taking progynova. I'm a smoker and over weight. In February, i was hospitalized for hypertension and high tryglicide. Now my tryglicide level still high and on medication. I wanted to resume hrt but affraid of the risk blood clot. So i beginning to consider spironolactone but i need some advice from other transgenders. Does spironolactone has the risk of blood clot? And does blocking male hormones makes my skin smooth? Thank you.

Regarding your questions I am most certain that other more knowledgable members will be along that can intelligently answer what you asked.

Of my limited knowledge about HRT, it's the estrogen that causes the risk if blood clots. Cis women taking birth control have the same issue. Smoking increases that risk tremendously. From what I've read it doubles the risk. When I started HRT my blood pressure was high as I'm a smoker also.
I don't know what options are available to you, but I chose to get estrogen pellet implants as it carries the lowest risk of blood clots. Injectable estrogen is the next safest, with oral estrogen being the highest risk. From my understanding, it's the spikes that cause the issue. The more level and constant the estrogen, the safer.
As for taking an anti-androgen like spironalactone alone, I'm not sure just how much feminizing you'll likely experience. Others may know better. I started Spiro at the same time as estrogen pellets. I do know that long term lack of both testosterone and estrogen can lead to other health issues like osteoporosis.

I've heard nothing but great things about Bicalutamide instead of spiro.
Though I still can't start HRT due my financial state I decided to quit smoking to protect myself from such problems in future. Maybe it isn't solution you looking for, but it is an option. I also lowering my weight (not because of HRT or something, but I have listhesis and doc forbid me from any exercises except bike and pool until I lose weight :D ).
Spiro is a diuretic used for Congestive Heart Failure to remove liquid from around the heart. I have been on it for 25 years. Eventually is will kill your sex drive and may give you man boobs but that's about the extent of it. You will pee a lot, As part of HRT the dose was doubled so now I pee even more often and the sex drive is nearly 0. It did take about ten years to start to effect my sex drive. I had never shot blanks until then. I have lost 80 lbs. and that has helped my diabetes, high blood pressure, I have had a previous stroke (tia) and I really thought I would never be able to take estrogen so I never pursued it. Then I had some truly great comments from Dena and HughE right here as Susans and it convinced me to proceed with HRT. Estrogen has been a life saver and as long as we monitor things I don't have to worry much. One advantage I do have is that I am on blood thinners. But estrogen has given a clarity my mind has not had since grade school when I used to play with the girls. And when I was seven and asking my mom to dress me up. Somehow I feel I am connected to those years and I understand now why I was that way. And have always been that way but I had to hide. I hope that helps. I'm sure others have had different experiences.
