Susan's Place Transgender Resources

Hi all,

I got a question I'm planning on changing my name and gender on my birth certificate in the next few weeks or so. I was just wonder do all i have to do is fill out the form and have the affidavit signed or do i need a letter from a physician also?

It depends on where you live. In the United States you could require a letter from the surgeon which you provide to the court and they issue the order to change your birth certificate. Other states have less restrictive procedure and other countries have their own standards. Googling your location+transgender or transsexual+birth certificate will normally provide you with the procedure you need to follow.

I took a quick look at the form for Quebec.  The only reference I can see to requiring a doctor's letter is if you have applied for a change of gender in another juristiction (whether you received the change or not), then you need a letter from a Canadian physician.

Thanks for your help that's what i thought. What was getting me was the part that says "Appendices and additional documents must be included with the form on the basis of each case. When the application concerns a minor person, you must, in particular, include a letter from a physician, psychologist, psychiatrist, sexologist or social worker authorized to practice in Canada or in the State in which the person concerned by the application is domiciled, who declares having evaluated or followed the person concerned by the application and who is of the opinion that changing this designation is appropriate." which I'm pretty sure is just for minors.
