How do I tell him? I don't know what words to describe that I have a wrong body or "woman" trapped in man's body.

Depends on what you want to get out of it. Do you want hormones, do you want a referel to a therapist?
In most cases a gender therapist is probably going to be more useful at this stage, most doctors won't prescribe hormones without a letter from a therapist. But you never know.
It sounds like you are ready to come out? Good luck honey! it is a heck of a ride.

well as a gender issue, I think it is best to find an experienced therapist. they will help you work out what to do next. Most doctors are not so up to play with the trans-gendered.
If you contact a local support group they may be able to recommend someone.

Welcome honey.
Some of the referals also depend what country you are in. Looking at the USA, UK, Aus and (of course :-*) NZ there appears to be a gender based therapist approach. That is you request your Dr for a referral to a gender specialist. If you are in a place that Drs can prescribe HRT without reference to specialists do make sure there are specialists involved. You need blood tests to track the level of boy and girl hormones. It can be quite erratic and you do really need to be monitored. You can get serious health damage by the wrong levels. As a MtF you need to have bone density checks. You also, not matter how strong you are, will need to talk to a professional at times. You may need to include family etc.
I think one of the reasons that the therapist approach is common is, well one it works, most of the time. It also grounds you. This is irreversible. There may be alternatives that will allow you to live a happy and gorgeous and successful life. There may be not. We have to find them.
Going physically from MtF or FtM, there is no magic wand. There are just days, weeks, months, years, of going for it. It may be fun, in many ways it can be, but there is also a lot of emotion.

Maybe a good way to go would be to just tell the doctor you believe you have Gender Identity Disorder, and simply ask if he/she would prescribe HRT for you. Most likely, they will decline. At that point, politely ask them for a referral to a gender therapist. Some T-girls have actually gotten their HRT just by asking for it outright. Usually, though, the M.D. will require a referral letter, and a gender counselor is your best bet to write one. That's the route I took and that most of us on here took.
The important thing is, you're out to YOURSELF! At this point, this whole matter may take on a life and momentum of its own. If it does, then there's a darned good chance that you truly are trans and should proceed with transition if that is what you want.
Those of us on here who were diagnosed as trans can basically agree on telling you that GID is like opening Pandora's Box or like a runaway train: Ain't no stoppin' it!
