Title: Informed consent for testosterone

As I was trolling the forum a few days ago I happened upon a thread about where other people were/are getting their T. I heard a few people talk about Fenway. Well, I was on their site today and saw that they had an informed consent form for testosterone. I was definitely intrigued but I'm not really sure how that works. I mean I know the basics i.e. you sign the form stating you understand the risks and blah blah. But, is that it? Just sign a paper and you're all set? I'm 22 and I live in New York(the state btw). So, I know age wise I'm fine. Could someone tell me the process of informed consent? I mean I thought about filling out the form on Fenway's site but I have my doubts as to whether or not it would work simply because I do not live in the same state. Can I just fill out that form and give it to my doctor? Or even better! Does anyone know of any places like that in or near Buffalo? I really wouldn't mind getting a letter and everything it's just that I've been through all this therapy and jumping through hoops stuff for about 5 years. I'm getting sick of it to be honest with you. I thought it would be easier once I turned 18 but well, I'm 22 now and nothing has changed. Anyways, any help would be greatly appreciated.

I go to Fenway so I can answer the questions about them there. They do require that you sign a consent form (which is just exactly that, a silly little signature) but you have to have a letter from a therapist as well. They then present your case before a board about a week later and if all goes well, which it 99% of the time does, you get your T.

In other words, the consent form is just something extra to the therapists letter that Fenway likes to do.
