Oral vs. transdermal HRT

Plain Jane

This may have been discussed before, but I would like to share my experience of the past few months with respect to the difference between oral and transdermal estrogen.

Needs a rather long introduction.

Before I started HRT back in October 1992 (on self reflection) I was so conflicted that I could best describe myself as a-sexual.

Then in October 1992 when I started HRT, I was put on Androcur (anti androgen) and estradiol (brand name Linoral). I don't really remember the dose of Androcur I was taking, but it pretty much did the trick in the anti-androgen department. That was a great relief, but it included no interest in sex, etc. A year and a half later (March '94) I had my SRS and was taken off the Androcur and the dose of estradiol was halved. However, libido remained pretty much next to nothing. By that time this had become the "new normal" for me and since I had been pretty much asexual before anyway, thought nothing of it.

During 2000/2001 I developed what I can best describe as a metabolic disorder which may or may not be related to HRT and SRS. Long story short, all the doctors I saw couldn't find a thing until I found this alternative medicine doctor. Whether you believe in alternative medicine or not is for a different topic, but I believe he (literally) saved my life. Anyway, in the early 2000's I was trying to stay alive and so the exact effects of the estrogen were irrelevant. In 2002 Linoral was taken off the market and I was put on Progynova (oral), which is estradiolvalerate (spelling?)

As my health (metabolism) gradually improved over the years, the "normal" for me was no sex drive, no real interest in doing anything to look nice. I have a feeling, now in retrospect, that the sex drive lowering effect of Progynova was stronger than Linoral, but I have no hard evidence of that. I had entirely stopped wearing makeup around 2001 because much of it somehow interacted badly with my metabolism (and it just wasn't high on my list of priorities). A real "Plain Jane". Then in October last year (2010) my doctor switched me from Progynova to transdermal patches. Reason being that as you grow older transdermal patches apparently carry fewer risks, since the dose can be much lower for the same amount of estrogen in your bloodstream.

Wow! What a difference! My libido is back, and I have come to the conclusion I am interested in men. That's some eye opening experience at 43, SRS+17 years. I am again much more interested in things like wearing makeup and generally looking and dressing more feminine than the more or less androgynous look I had slowly come to wear (without giving it much thought, mind you). (Aside: finding makeup that doesn't interfere with my metabolism is a slow road, but I am making progress).

I have since read that the best way to make a woman androgen deficient is to put her on oral HRT for a while. Which leads me to the conclusion that the switch from oral to transdermal estrogen has caused my testosterone levels to come back up closer to what they would normally be in a woman.

The transdermal patches have the disadvantage that you are pulling remnants of the glue off your skin after removing the patch, but the advantages I have seen far outweigh that.

So anyone else have a similar experience in switching from oral to transdermal?

I'm not a fan of either. I've tried everything over a 12 year period and I think shots or implants work better for me.

Can not do patches as I get contact dermatitis.  A horrible rash.  Never did injections.  So I am still oral Estradiol.

I've done the trifecta.

I started with transdermals.  After a couple of weeks I started to see slight physical changes and in a couple of months I had noticeable breast growth.  I worked around the skin irritation by relocating the patch daily.  I was self prescribing then and common sense got the better of me so I stopped for over a year.

After I got my HRT letter I went to see my new doctor.  My first blood test showed my E was at 57.  He put me on orals and I returned two months later for another blood test.  My E had dropped to 27.  Phsyical changes were nil.  My doc was baffled.  I finished the prescription and then returned to transdermals and the physical changes resumed.  When I went back for the next blood test my E was 24 even though breast growth had increased.  Again my doc was baffled.

Then we went to injections.  There were no noticeable physical changes after that but my blood test results were "perfect", according to my doctor.  I didn't care so much what a piece of paper said, I wanted to see the results on ME!  And it wasn't happening.  But I stayed on injections for a couple of years until I ran into a serum that caused a big swollen knot right where I did the injection.  My doc couldn't get the other stuff.  Over time I injected less and less because of the reaction, which was lessening each week but it still bothered me.

Finally I got tired of the injection thing and went back to orals.  I let them sit under my tongue until dissolved.  My blood test results are still "perfect".   As far as physical changes, pretty much everything I can see happened with transdermals.  I've done a ton of research and have found only one person who had the same reaction as me, Zoe Brain.  Maybe that means I'm smart!

I self medicated on pills on and off for years, now on official patch and it keeps falling off, the current one is being held in place by sticking plasters, and its not the first time I've had to do this either

my skin is like teflon, nothing sticks to it, especially those climara patches. One day in florida heat and its coming off. It gets itchy too but nothing I couldn't stand. So yeah, estrofem pills for me. I will admit I feel like the patches work best too, when the patches are sticking. I might try that vivelle dot when I run through my pills and just tape that sucker on lol.

I'm on transdermals but not a lot has happened physically.

Going to have a BA at some time.

The junk is so destroyed from previous events (not self inflicted) that I'm not sure if there is much useful tissue left to use in an SRS.

Oh well

I have done all three.

I started with the pill and with in 3 months i started feeling the changes like the itchy breast, headaches, muscle cramps and euphoria. I switch to the patch after about 10 months and my body reacted in a big way. I had significant breast growth, face changing and other physical effects. But, my skin started breaking.

I'm currently on injectible estradiol and hoping on being able to go to implants soon.

I prefer Climara, but Progynova is cheaper.  So I have been on both.  If you take the pill sublingually, you avoid having it processed by the liver, and it is no more harmful for older people than patches.  But, I think the patches keep an even level of estrogen transmission for days at a time, whereas pills are going to give you spikier levels.  If you look at some doctor recommended regimens, they have both transdermal and sublingual estrogen at the same time.  If I could afford it, I would be using a patch, twice daily pills, and a transdermal gel.
It's easier to change your sex and gender in Iran, than it is in the United States.  Way easier.

Please read my novel, Dragonfly and the Pack of Three, available on Amazon - and encourage your local library to buy it too! We need realistic portrayals of trans people in literature, for all our sakes

A lot of patches fall off but try Evorel patches as they seem to stay on.

I guess I have been lucky, so far no problems at all with patches. I use Systen (which might only be available in Europe?)

I've been on all three, but mostly on patches over the years.  I place the patch on the side of the abs above the waist.  I had problems with the patch worn lower as it would be pulled off by panties or even pants.  I found the patches had fewer emotional ups and downs.
