I'm in the middle of filling out an application for a pharmacy, and they are asking  "Are you now, or have you ever been known by any other name, or have you changed your name (first or last)?"
I should put the name that I go by in public and at school, right?
My birthname is obvously female so I'm kind of nervous about writing my chosen name.

I ususally put... birth name''shaun''...also ive been going by shaun for over a year now so plenty of ppl now know me as shaun...if ur not using ur preffered name u could just put ur legal name and explain that u prefer to go by such and such at the interview or whatev
