When I go swimming I wear swim trunks and a rash guard (binder under it). You can't have a normal t-shirt in the pool because there's a potential for the shirt to rise up and to drown. At least that's what I was told from a lady that worked at a water park. They should sell them at any sports store.

Clothing that's not made for water can actually weigh you down when wet, the color could also bleed into the pool.

A rash guard is fine.  I have a cis-giendered friend who had surgery to remove tumors in his lungs as a child and he was always very self conscious the scars and never went shirtless.  No one has ever given him a problem at the gym because he was wearing a rash guard.
