Name change form question

Hi,

I need some assistance on one of my name change forms. These might be dumb questions, but I've looked online trying to figure them out for a while and I can't find anything. I'm in NJ, and I have to fill out a Civil Case Information Statement. One of the things I have to fill in is the caption (name) of the case. Obviously, I'm the plaintiff since I'm filing it, but I'm not sure what to put for the defendant. Would it be the state of NJ? That's what I'm thinking but I'm not sure. Also, the name change forms have a couple of places to choose which pronoun to use. Do I have to use the ones that go with my legal gender, or can I choose male ones, since they reflect my true gender? (After all, I am changing my name to a male one.) Thanks for your assistance.

I'm assuming you're using the right forms because I don't see anything about naming a defendant which is for actions against another party.

One should use ones legal gender and pronouns (...) as the state knows it in birth certificate record.
