Trans-friendly GP in San Jose/Palo Alto area?

I'm hoping to find a transsexual-friendly General Practice or Internal Medicine doctor, somewhere between Palo Alto and San Jose.

Just for annual checkups, prescriptions, etc.

It would be great to get a personal recommendation from someone in the area.

Any suggestions?
