But Lara, if he is an artist, then why did you need to do a revision on your urethra and and problems with peeing? I am at the final stage of deciding and I am in a real agony. Dr. Djinovic in Belgrade does a 7 hour surgery and after that you don't have to do something again (cost, 17.000), but Schaff needs 20.000 euro and also requires 2 surgeries. Do you know what does this mean? Do you have to pay extra for the second surgery? Are you left with unbiguous genitalia after your 1st surgery, only to be corrected at your second?

I had an complication which was solved fast and with a perfect outcome. Every grs surgeon has complication but only des girls and Surgeons speak about that. I know a suporn girl which hat three revisions done and still is not happy.

His method is very complex and he thinks the results will be better with two surgeries. The second one is usually more a cosmetic surgery. I wanted smaller outer labia and wanted to be a bit less  tight at the entry. The genitalia will work fine after the first surgery.

His philosophy is to create a deep and perfect looking genitalia with a fantastic feeling.I did not lost any feelings down there! There is not hair inside without any electrolysis. It looks like every CIS girl genitalia, even for a doc during a gynological examination.

As far as I can say only Suporn and Schaff can create something like that.

could you please describe the shape of the clitoris that dr. Schaff made in your case - flat or bump? I have only seen one result from dr. Schaff personally and in that case, the clitoris was flat instead of a bump (or a knob or a hill). All other surgeons I know, including Suporn, make the clitoris like a bump.

It is flat and looks very similar to my CIS wife's clitoris. It is covered by the clitoris hood.

I'm not from Europe and am paying out of pocket. Do I still have to wait 3 years? If so, maybe I can learn German in the meantime! I don't mind ~1.5 year wait tho.

It's interesting that many thought it was good, tho someone else also found hers weird. Personally I prefer inner labia like Suporn's. Lack of info on Schaff and his quality attested by only a few of girls on here make me think it's safer to go with Suporn... I believe he has a long waiting list mostly because he's one of the best in Europe and also European girls get covered by their insurances.

(which Dr. Schaff always claims he is not doing - the cause being that he says using a scrotal graft will not make it a classic PI any more - yet he inverts the penile skin to line the neovagina - so from my terminology it is still a PI - yet an improved, advanced version of it)

Well there are many people who are happy with a result fom Brassard etc.

Inner labia ending are within natural variance, there is a lot of variance within vaginas.
Its possible to look up wall of vaginas, there is a lot of variance there.

And it may also be possible to have a revision later, surgeons might be asked for it.

Well some people consider a revision after some time to possibly construct some finer details.
Meltzer for example works in two stages, and there are people who are happy with the first stage, and also people who come there for a second stage from other surgeons.

I didn't say anything negative about Dr. Brassard at all.

It was just to mention that there are other surgeons doing modified pi where many people are happy with.

One main reason being, that he has only performed roughly 250 surgeries with his "combined technique" - a number which all other world class surgeons surpass by far - so their routine and experience is far greater.

Thanks for the valuable info! I'm a bit surprised to hear negative points about Schaff. I've only seen a picture of one work (after a revision) but that looked quite good to me.

I had 6 consultations at Dr. Schaff's office - and I have seen more than 5 of his surgery results in person.

From all the German surgeons I personally believe him to be the best by far.

Compared to the Thai surgeons he is no match at all for them in my opinion!!!

First of all - his clitoris doesn't look like a clitoris at all - from all the results I have seen so far.

In contrats to a post in this thread he doesn't guarantee lubrication - at least in the form that there will be no need for additional lube to have intercourse.

1) Dr. Schaff has a 2 stage surgery - the best Thai surgeons perform a 1 step GRS/SRS

4) The scarring after Dr. Schaff's surgery is more visible and prominent due to the technique used. Should you prefer to be shaved, than these scars will be quite visible and other surgeons achieve a way better result. Neither my result (Dr. Chett) nor Dr. Suporns or Dr. Sanguans results have visible scars in the mons pubis upper vulva area as Dr. Schaffs girls do.

5) The clitoroplasty is far from a genuine clitoris. Seeing PAI results ( like the one of my fist client) this can be done way better - Dr. Schaff's clitoris looks like to small folds of skin running down on either end of the upper vulva opening, being of dark, purple, glans penis color.

6) The patient care in Thailand - and as I have heard of Dr. Brassard and Dr. Bowers also - is way ahead of the patient care you will receive in a German hospital.

I decided to not go with surgery at his clinic due to the fact that:

A) a hairless vagina is not guaranteed

B) the routine and experience is far, far less compared to the Thai doctors as explained above

C) the scarring and the clitoroplasty is far from what I was seeking

D) the price is more than twice as much as in Thailand

E) I know German hospitals - and the one in Munich, where Dr. Schaff has his patients also - and I would never, ever trade my Thai hospital experience for anything - neither do any of the women that came  with me to Thailand for SRS ( all German)

F) I don't like the 2 step surgery approach

Yet - if Germany is your limited area of GRS/SRS Dr. Schaff is so far one of the best choices you can make.

Also Dr. Hess from the University clinic of Essen is performing GRS/SRS with the Dr. Schaff technique.

He got training at Dr. Schaffs clinic in summer 2015 and started doing surgeries with the new surgery technique in late 2015.

Benefit here:
No additional cost! (German patients have to pay a 2500.- Euro extra fee in addition to the health care coverage to have GRS/SRS with Dr. Schaff)

I personally also know Dr. Hess and he is genuinely interested in improving the situation for transgender women in Germany.
So far he already has performed quite a few surgeries using the Dr. Schaff technique and his patients seem very happy with it - but I have so far not met one in person.

I cannot agree to man of your points. I had surgery there, a good friend had surgery with Dr. Suporn and I saw results from PAI and other surgeons too. I am not sure where you got some informations from. Some seen just to be the results of a wrong translation?

Just a few points:

- I get wet like a CIS girl
- It looks very natural, evem some MD thought It is a CIS vagina
. Healing time and pain are a lot faster and lower like with the Thai docs.
- Without revision Thai results are often not satisfying from the girls I met. Some had a revision with Schaff because of problems or because of the look.
-  I had a wait time of 6 months for the surgery with Dr. Schaff
- I do not had an extra fee (I am a German patient)
- The hospital in Munich is a public and compared to other publiuic in Europe or the US one of the nicer hospitals. I had FFS and VFS in the US and Spain to compare with.
- Lately I had a small revision because I wanted a porn look. The older scars are not visible. My new gynecologist said it looks like a CIS Vagina with an small surgery to reduce the outer labia. I had all hair removed on the outside after surgery. Because of his technique mons pubis must be scar free.
- My vagina is completly hairless with any hair removal before.
- He performed 500+ surgeries.

Who has given you this informations. Lot of information are just wrong. Are you sure you have seen results from Dr. Schaff? Most of my friends had surgery with Dr. Schaff and the result is the same. As I was 46 years at surgery time my result is for sure not the best but looks very natural. I already gave some readers here a picture of my result.

Since I have been to countless hospitals around the wold,  also accompanying customers worldwide, and having visited some friends at the Rotkreuz/Frauenklinik Hospital in Munich for their GRS with Dr. Schaff I can honestly say that the care, privacy and quality of the hospital care is no match for PAI, Dr. Sanguan's PPAI, Facialteam Marbella or Dr. Chettawut or the Yeson Voice Center in Seoul.
It is German standard. Again-if you have privileged private health care, which is what you automatically get when you come from outside of Germany, then care and quality us much better than for standard patients. Also the overall costs of the 2 step surgery run around 30 000.- Euros for both surgeries. That is far more than all of the most expensive Thai surgeons and even Dr. Brassard or Dr. Bowers.

What about the quality of care and facility at Suporn's? Does it compare to the ones you listed above?

Also are you saying that patients on private insurances will get better rooms and quality care after SRS with Dr. Schaff vs German/Austrian patients under the national health plan? Or is it just the overview of German healthcare in general?

I didn't say a Dr. Schaff vagina CAN be hairless,  I said they don't guarantee that!!!! The Thai surgeons do. For me that was enough reason to not trust in his work.

I don't think Suporn actually guarantees no hair, but it is very rare to have hair post-op. I've only heard of it a twice as far as I can recall. The women in the above pdf has hair post-op.

I have seen SRS/GRS results from women who claimed to be very happy and thought their genitalia resembled a genuine one - and I could not see any similarity at all. For me those results would have been a reason to end my life.

That is why my advice is: 
PLEASE MAKE SURE to get as many result pictures of the surgeon you plan to go with as possible - the best thing is to see a result for real - and make sure that your requirements meet the ones of the person you talk to.

Yeah, that's true. It's not easy to travel around the world (Germany, Thailand and US/Canada) to have a consultation in person and take a look at photos but we are talking about your vagina.

Oh my, if I knew earlier I'd have contacted you. I was pretty terrified of going alone, I'm generally pretty lonely and of the few people in my life none had the time to come along. I'm german too but surgery in Germany was never an option to me. How would I have found out about you if I had looked earlier?
Pretty cool idea to offer such a service in my opinion. Plus you seem knowledgeable, I keep doubting my choice to go to anyone who isn't Brassard or Saran, dont wanna imagine how bad it's gonna get once I have to take a break from HRT prior to surgery.

I've already made my decisions and my surgery is only 3 weeks away. Back when I was making my decision on which surgeon to go to, Saran didn't even occur to me as an option, but as opposed to Chett whom I'm going to, you find almost no bad reviews or info on Saran and the results someone has shown me looked amazing. Or at least I couldn't find any anyway.
You seem very pleased with Chett, which is partly why I said having contacted you earlier might have put me more at ease for not choosing Saran instead. Sanguan doesn't strike me as a better option personally. Brassard sadly wasn't an option due to lack of cooperation from my therapist, so where Chett let me book a date without WPATH letters, Brassard required them upfront.

I still got some patches laying around but I went with pills anyway as the patches often didn't stick well, but I'll definitely ask Chett if it's okay to use patches. No dilation scares me as much as being off hormones for a month.

Yavanna, there's a German trans site that has photo's of Dr Schaff's work. However I don't recall the name of the site and I'm told its difficult to join even if you're German.

I think the other thing to consider is that even if you do see a photo or two you won't know how representative it is of his work. There's quite a few of Dr Suporn, so the risk in knowing what you're getting is less.
