what NOT to say to the Endo

1st appointment next week. I'm in my late 40's; smoke and drink. I'm looking to be androgynous with nice nipple areola development and of course the mental changes.
My testicles are way too large for my comfort. I don't plan on staying on E for an extended amount of time. I have to support my wife and work at one of the largest IT/PC companies in the US. I can get away with A's but probably not B's unless a sports bra can keep them hidden with the right clothing. I live in Texas so it gets really hot in the summer. I just want a low dose of E and some spiro. I'm concerned of telling her too much of my intentions for long term. Who knows, I may say WTF and keep on them if they don't make me too scatter brained or make my breasts really huge. My family has B-C's.

Your endo will start you off slowly anyway, especially since you smoke as the estrogen will increase your risk of clotting. Just tell her you want to start off slow and that you aren't looking to fully transition just yet.

Most, but not all will withhold HRT until you stop smoking. The risk of DVTs is just too high.  :)

What not to say I believe would be to show any hesitance or indecision.

I definitely want patches and a low dose. Don't need a stroke. Can't afford too much fog anyway.
My wife and I agree if I don't at least try this I will continue to blame everybody I know forever.

Not quite sure what you mean by fog or scatterbrained. I've been on HRT almost two months and find my thoughts have calmed somewhat. Progesterone does, on occasion, cause some euphoria and lightheaded feelings but those days (and the drowsiness) are getting fewer it appears. Most of the time I just get a general sense of overall well being from it.

What not to say?  Anything that would be a lie - esp. RE: your medical history (drinking, drugging, smoking, liver problems, clotting problems, etc).  Just be honest with your Endo - think about your current/short-term goals of HRT, and be honest about your long-term goals (if you know those already).  The Endo knows that things can change along the way (esp. the long-term goals), so there's really no point in omitting what you're aware of at this point in your journey.  Keep working with your therapist and your wife to hash out your goals.  Long story short - just be honest and open with those involved in your journey.

I"m also from Texas, so I know all about the heat (from the Gulf-coast muggy to the West coast dry/oven heat.  I've not had an issue with my breasts - mine are  developing (slowly...38AA/36A).  I do have an athletic frame, so they somewhat merge into my overall physique, but they jiggle up flights of stairs, and I can tell when someone is staring at them.  When I need to squash them down I use a tight undershirt and a slightly bigger button-up at work; on the weekends, a sports bra (with wide bands) under a T-shirt.  Since TX loves their AC units, anytime I'm inside (coffeeshop, store, etc), I usually have a thin sweater to cover up even more. 

I've never understood the 'brain fog' insinuation.  If that were true, than that would seem to justify a perception that cis-gender women are intellectually sub-par compared to cis-gender men, which is patently false.  Maybe cis-gender women analyze problems differently than men, but that's not brain fog.   Nevertheless, I've not experienced any decline in intellectual capacity since starting (almost 2 years), and I work in a highly technical/analytic field.
You have suffered enough and warred with yourself - It's time that you won.

Brain fog here is mostly defined as the storm that controls us pre transition. It is a competition of your two personalities leading to anger, frustration, isolation, etc. It has nothing to do with a decline in intelligence at all.  :)

I never understood the brain fog thing. I experienced only calmness and an over all feeling of  well being.  I still feel this way after over 22 years of HRT.
I have been on estrogen and progesterone  for 24 years and sprio off and on but not for about 8 years. I have not totally transitioned yet but since  my breasts are very noticeable  36 I it is pretty much impossible not be seen as female. Just went full time. :) Yes I still do yard work!

My brain fog vanished on HRT also I experience an unbelievable amount of calm and well being. I never feel the floor is going to drop out anymore.

I originally wanted low dose but then requested full dose and never looked back. I just transitioned to injections and love it.

Seriously, I am clean, sober and smoke free. I had a hell of a time but it can be done.

With the doctor, just be honest, decisive and know why you want HRT.

Some doctors will prescribe non-oral bio-identical estrogen to people who smoke as this way, DVT is much less affected. Smoking is not absolutely a contraindication for all doctors who treat transwomen. But, no one will argue that smoking is BAD and should be stopped ASAP. Better for you!
