So one thing I was absolutely prepared for was that my genitals have shrunk after several months of HRT.  My wife has noted that, on the rare occasions it still becomes erect, it doesn't fill in at the base as much, and she's worried about causing permanent damage.  Are there any work arounds for someone living with a spouse that very much enjoys penetrative sex?

Not too much you can do once the mone's start kicking in. Might want to look into a strap on. As in permanent damage what does she mean? After a period of time you will be sterile.

I'm 9 months in and that part of me no longer gets erect unless forcefully made to which takes a little more time, spontaneous erections are gone.  Although I will state that during the night there are times it will be fully grown.  As for penetration, good luck, I doubt you'll be able to keep it erect for long.  Also you will 90% of the time have dry orgasm or minimal fluid come out.  I only use mine for bathroom purposes as I've learned to achieve my orgasms in the way women do.  :) MmmHmm!  Which are so much better.  As someone else mentioned you will eventually need to look into strap on or sex you, sorry.

You can consider toys if you loose all function.  I can tell you that I am almost 1 year on hormones and when my wife stimulates me as a woman I get really excited and I do still have functionality.

by permenent damage, she's worried about possibly bending things in such a way that internal parts might be damaged.  I've been sterile and 'dry' for several months now.

Sorry gonna be blunt here; it Just about kills your ability to create sperm, you can't ejaculate (well maybe a trickle), feeble or non existent erections, loss of libido, retraction of testes... All permanent changes.

In my book thats a big Yay  :-*

Before starting HRT you need to be committed to the outcomes, after a couple of months of E the effects start to become irreversible...

It's true what they say.. "When you go through HRT, you are chemically castrating yourself."  ...I'm not sure that is 'happy dance' worthy, but I guess I can come to terms with that.

After 4 years on HRT (Spiro and estradiol) and at age 62, I can say the erections are still there and is not spontaneous and not as large as before. It needs stimuli to erect such as a good dream or a nice, single woman in my case. There is ejaculate, but it is a clear or a pinkish clear liguid called seminal fluid and not semen because there is too little or no sperm involved. My Dr. suspended the spiro dosage 6 months last year to recharge my minerals it was flushing out. The thing roared back to life on the estradiol alone during that time! Don't worry about bending and breaking anything! Your wife should be stimuli enough, keep at it.

This sounds like a normal thing since it's all shrinking. In my case I cannot achieve an erection ever (but don't want to either). Vibrators are the way to go for stimulation once it changes down there if you ask me.

Strap-ons are super fun :)

I'm a tad over four months in and wondering when I'll get hit with the loss of functionality. I uh... kinda like using it still but if I lose it I'll be okay.

^ are you on t-blockers, if so it's not uncommon for them to take 6 months or so to take effect

Some functionality is there, after some work a soft small erection occurs, you can still get orgasm, but its dry, different, not a male feeling (orgasm) anymore. Medically I am castrated... next step would be to remove the need for T Blockers permanently (please girls correct me if I am wrong) by GRS (SRS) etc.. Oh another thing, I seem to have lost that "male" aggression trait, now I get teary and emotional (re-watched Blood Diamond last night, was a blubing wreck at the end... happens so much, in fact I blub quicker than my SO) I love it  ;)

But I am happy with my outcomes, boobs, a waistline, soft skin, softening of my old facial features. I just need to drive myself fully over the line (I always question myself on why did I do it this way round???)

Immortal words from my endo when I first presented to her (she is lets say ....eccentric!) my T levels came back and her comment was. "You weren't much of a man anyway, don't know why you bothered", I was sterile and non erectile within a month.

Well, I've been on just E and Finastride for almost 15 months now.  It still works but yes no sperm and erection takes work. I have come to terms with it however and just accepted that for now I certainly don't mind it as long as my partner accepts that it will change. To which so far she has been great about.

mine has shrunk so much I have a terrible time peeing whether sitting or standing. I can't get a hold of it to point it in the right direction, but it's cool because it's like a little oversized clitoris

I'm a little over 8 months in and my T levels are literally undetectable. I've lost spontaneous capability, but despite that I have zero problems with functionality when I decide I want to use it, so it seems very much YMMV.

It really does vary and I thought it was HRT affecting my function but it turned out to be one of the combo blood pressure meds I was taking. So the doc changed it and took away one half of the combo and upped the other one, and added spironolactone. What a big difference, literally. I don't have spontaneous erections anymore but when I want to use it, I can.

I lost most of my ability to have full and spontaneous erections after my prostate removal. The advice from the Dr was use it or watch it shrink. It is possible to damage the penis when attempting penetration with an insufficiently erect member--Google Peyronie's disease. Post-prostatectomy patients are prescribed a low daily dose of Cialis to improve blood flow, since we may no longer have the nocturnal tumescence (morning wood) necessary to provide blood flow to the erectile tissues. Options for continuing penetrative sex include Cialis and Viagra, a pump, and, the nuclear option, injections of bimix and trimix, which can literally raise the dead.

For my wife and I? Meh. Good riddance.
