Buying shoes for larger/smaller feet

It turns out I'm a size 11 1/2 ish in women's shoes.  I can fit into size 11s and 12s as well for most of the shoes.  But a lot of stores don't carry too much above size 10.  What kinds of places do y'all shop for shoes, especially for those with feet that fall outside the typical range of shoe sizes?

I've got two pairs of Karmen pumps from Payless, one in Patent Black size 12w and Red in 13r.  The black patent are a little tight and the red ones are a little loose.  I've spent a few hours in the black pumps and they're starting to loosen up, the red pumps will need some toe stuffing so my heel fills them.  Payless carries up to 13w in many styles, but you'll need to find a store that commonly carries these sizes.  Another option is Zappos.com if the shoes don't fit, they have a free return policy.  I'm also looking at clothes through Zappos, there's a lovely long black dress I would love to have for orchestra concerts.

I'm an 11.5 as well, which is unfortunate because half sizes are really rare above an 11. I can usually fit into an 11 though, especially if I'm not going to be on my feet all day.

I've ordered from Zappos several times and I've always been been very happy with their customer service. Sometimes items will ship the same day (!) and shipping is really fast. Turnaround times are really good on returns as well. I've put something in the mail as a return on Monday and had my new pair by Saturday.
