Top surgery for chubby guys

Last summer I had a breast reduction in the hopes that it wold resolve my chest issues without breaking the bank (insurance covered it, so huzzah for no more neck/shoulder pain!) I ended up with a B cup, which, while a definite improvement, is still larger than I'm really comfortable with. I'd like to get my chest "finished" at some point in the next couple of years, but I really don't want to end up being too flat. I'm a bit on the chubby side and that's not likely to change. I've got the same build as many of my male relatives, and learning to be more comfortable with that has been one of the most positive parts of my whole "gender thing." Part of the reason I went for the BR is that I hoped it would get me sufficiently small without being totally flat. I feel like having a totally flat chest wouldn't look natural for my body, and would feel nearly as off to me as what I've got now. However, I talked to a couple of surgeons even before the BR about top surgery "without being totally flat" or "to match my build" and they either seemed confused or in one case flat out unwilling.
I was wondering if any bigger guys here have suggestions about how to talk to surgeons about this to get a good result? Is there anyone in the US who does a particularly good job of tailoring surgery to body type? I'm kind of hoping I can see the same surgeon I did before since he knows how my previous procedure was done and did a nice job of it, but he's not an "official" FTM surgeon so explaining it to him seems even more difficult.

Perhaps one reason why you are getting confusion from surgeons is that they wonder why you want to leave any mammary tissue in. As long as there is mammary tissue, you are open to breast cancer.

If you don't want to look completely flat chested, try working with the weights for a while. Bench presses while laying flat, on the incline and on the decline will help build the muscle in your chest, rounding it out. You can do that either after you have completely recovered from chest surgery or before you have the rest removed. Breast tissue looks very different than muscle tissue, so the new muscle would not be mistaken and removed with the breast tissue.

These days, a good surgeon, one who does a lot of FTM chest surgery, can contour your chest so that it's in proportion to the rest of you.  Take a look at Transbucket.com -- it has before-and-after photos of guys who've had all kinds of surgery, and most of the results are pretty amazing, even for bigger guys.

Definitely check out transbucket.com for photos - most surgeons are contouring things so it looks realistic and are not completely flat.  Also, developing the muscles of your upper body will create the shape of a male-appearing chest (and help with surgery recovery, in general!)

I'm skinny up top and Medalie does liposuction to get rid of the fat. Maybe it is just a matter of not getting contouring lipo done.
