Hey people! So I'm going to be filing my name change papers soon, but I need my birth certificate number. I live in New York. I'm not sure if it's the "medical records number" or the number in the bottom right corner that's not labelled that follows this pattern: 0000 0000A 00000. Any help would be much appreciated because I have no idea which one to put on my petition.
