FTM questions about getting T and therapist?

I am from America - "Bad" insurance so very unlikely they'll cover anything. So let's just say I don't have ANY insurance.


1. How much is T?

2. How long does it take to get a prescription? (i.e. How many therapy/doctor appointments etc.)

3. How expensive are the therapy/doctor appointments? (Blood tests etc.)

4. How long does a vial of T last till you need to buy a new one? (Shots)

5. OVERALL, how much are the average expenses from the start of the first appointment, to getting the first dose of T?

Hi there,

Understand that some of this will vary slightly by the area you live in, dosage, etc. If you're paying 100% out of pocket, doctors can set their own prices and many will be willing to work with you. I had "bad" insurance when I started my transition, and they were still willing to cover my doctor visits and blood work. So you may be able to get something covered in that regard. Now to your questions:

1. How much is T?
- I pay around $75 for a 10ml vial, (24) drawing needles, and (24) injecting syringes. This lasts me around 4-5 months. I pay out of pocket for this, because my insurance will only pay for 1 month at a time and it is more convenient for me to pay for a larger vial than to drive back and forth to the pharmacy as needed.

2. How long does it take to get a prescription?
- It depends where you go. I found an LGBTQ clinic in my area that follows the informed consent model, so I was able to come in one week, speak to a doctor, and have blood taken, and come back a few weeks later to get my prescription. I had no therapy prior to starting T, but I transitioned at 25 after knowing I was trans for close to a decade.

3. How expensive are the therapy/doctor appointments?
- I only had therapy for my top surgery referral. I paid $320 for 6 sessions and a letter with an online therapist. My copay for my doctor's appointments is $30. I pay this every time I see her. If I did not have insurance, her office bills visits at $200. But they work on a sliding scale, so it would likely be lower than this. Blood tests are done prior to every doctor visit. With insurance, this costs me less than $15 each time. Without insurance, it is $550 for the tests my doctor orders. The first time, she ordered 3 additional tests so it was a little more expensive. My doctor requires that people see her quarterly for their first year on HRT, so 4x for the first year. The second year is biannually, so 2x per year. After that, it is once per year.

4. How long does a vial of T last till you need to buy a new one?
- This will depend entirely on the size of the vial and the dose. I haven't tracked mine from start to finish ever, but I use the large vial and it generally lasts me 4-5 months. They give me enough syringes for 24 weeks/6 months and I always have extra by the time the vial is out.

5. OVERALL, how much are the average expenses from the start of the first appointment, to getting the first dose of T?
- It is going to depend what options are available to you. If you can find someone who operates on the informed consent model, it will be cheaper because you won't have to pay for therapy. But that isn't often available in smaller communities. In my experience, with insurance, no therapy:

- Parking at doctor's office, first visit: $4
- First appointment copay: $30
- First blood tests: $36
- Parking at doctor's office, second visit: $8
- Second appointment copay: $30
- 10ml vial of T, syringes: $75

I started this process while I was still in the military so I had to pay everything out of pocket because although I had insurance, I couldn't use it for fear it would end up on my military records. When I got out of the military I didn't have good insurance and I was working a minimum wage job so I know how it feels to be worried about the cost.

1. How much is T?

For me, T costs $25 for a 1cc bottle, which lasts me a month. The injection supplies are about $3.

2. How long does it take to get a prescription? (i.e. How many therapy/doctor appointments etc.)

It took me 5 therapy appointments to get my letter, after that I got my prescription at my first medical appointment.

3. How expensive are the therapy/doctor appointments? (Blood tests etc.)

My therapist cost $100 per visit and my doctor cost $160 per visit. I have an appointment with my doctor every 3 months. As far as blood testing goes my doctor called around and found the cheapest place in town for me to get just a testosterone test. The high prices were over $200 but the lowest price was $55, it pays to shop around a little. I have bloodwork done every 6 months.

4. How long does a vial of T last till you need to buy a new one? (Shots)

1 month

5. OVERALL, how much are the average expenses from the start of the first appointment, to getting the first dose of T?

Month 1: Therapist appt 1&2 $200
Month 2: Therapist appt 3&4 $200
Month 3: Therapist appt 5     $100
Month 4: Doctor Appt, bloodwork, and prescription $243

So total for me was about $743.

You can spread those appointments out more if you can't afford the monthly cost, or you can put more appointments per month if you can afford it and you want to start faster. It might seem like a lot, but I promise you that you can do it. Even on a minimum wage salary.

If you go to GoodRx, you can get a discount card for Walgreen's (or for other pharmacies, but they have the best price) -- I just paid about $45 for a 10ml vial of T. The cost per month works out to not much more than my insurance copay. I hate having to go back every month to get another 1ml vial, so I'm happy to pay out of pocket.

Thanks for this Tysilio. I always forget about GoodRx. The pharmacy in my doctor's office doesn't take it, but I could save about $30 by stopping elsewhere on the way home it looks like.

You can also find therapists who will write a letter right away if you are certain that you are transgender.  These are often people who feel that there should not be any sort of "gatekeeping" for adults who are transitioning.  Not saying that therapists aren't super helpful--I go to a gender therapist even though I got my letter in one session--but this IS an option in some places if you find the right therapist.  There is one in Dallas who does it.  So if you have been aware of being trans for awhile or having been living as male and just haven't transitioned medically, there are sometimes options that don't require 4-5 sessions of therapy.  Just as an FYI.
