Unfortunately there is nothing you can do with hand or feet surgically to make them smaller. I am a US11 and love heels but being 6' I try to not wear heels all the time. I find that flat shoes make my feet look longer so I try to at least have a little heel.
Wearing trousers that go down to below the anchles and a not too narrow also make the feet look smaller. DON'T EVER wear shoes that draw attention, like orange pumps (geez I miss the 80's

). If you have a wide foot you need to take that into consideration too when buying shoes.
I think it's not always the size of the foot but the way it is presented. If you foot looks feminine it doesn't draw that much attention to itself.
There are also behavioural matters to consider. For example, don't sit with your legs crossed with one foot way in the air... it's like saying "hey everyone, look at my foot".
With the hands, the only real thing you can do is grow your finger nails as long as you can feel comfortable working with. Chose nail polish colours carefully so you don't draw attention to them. Don't go overboard with rings. If you wear a watch, don't let it hand around your wrist, I prefer to wear mine 1-2" up from my wrist. Like in this picture
