Who do I talk to about hormones? Gender change on ID? Name Change?

I've already started my social transition for a year now, but as far as medically and the government, I haven't started anything. Who do I go to first about getting hormones? My family doctor or a therapist? Or would my doctor refer me to a therapist, or psychiatrist or etc. And how to I change my name and gender legally? When I get my gender change I can use that to get my name change and with a fee? How long would I have to wait to start hormones after I've seen my doctor/or other? I just have so many questions  :/

Your doctor is the place to start for HRT, they can refer you to a therapist and an endocrinologist. Depending on where you live and your age, you may be able to get hormones on informed consent: https://www.susans.org/wiki/Informed_consent
