Giving you an anti-androgen alone without E puts you in menopause where your mood, sleep, energy level, cognitive function and health might be adversely affected. You should ask your doctor why they do this.

Bio-identical estradiol has not been shown to adversely affect the liver, especially at doses usually given to us. The problem is more the risk of clots although this has been especially observed with non bio-identical estrogens.

Deeper in the rectum, the medication is absorbed in the portal vein and goes to the liver before entering the blood so absorption would be MUCH less and more similar to taking it orally. Also, whether estradiol in pills can actually be absorbed this way is debatable and has not been studied and verified.
