Insurance requirements help (MMC Healthfirst NY)

Hi all,

So I am looking to get top surgery I know what surgeons I am looking into and everything but my insurance has been a little withholding on what exactly I need to provide in order to get the surgery covered. They pretty much told me to just seek out a doctor and form a "relationship with them" and ignored my questions on what exactly I needed such as letters etc. I was wondering if anyone who has had MMC healthfirst could help me out and list the requirements (what letters are needed etc for each surgery as I do not want to set up consults until I have all letters in order and have fulfilled all requirements). My main worry is how much therapy would I need before top surgery? As I haven't found a respectable therapist yet under my plan in the long island area and the ones I have found are all too expensive out of pocket or too far away to travel to.
