It depends what you want really. I have to say, I didn't notice your Adam's apple until it was pointed out, and it doesn't look like the silk scarf route is your style. If anything, it might hinder you as a completely out of place element.
There's no of course about you having to wear make up, unless you want to.
I know several lesbians who look pretty much the way you do in terms of hairstyle and piercings and lack of make up, and I wouldn't have you down as the male if you were in a line up with them.
You have a narrow face and delicate cheek bones, good eyebrows, and a normal female hairline. Without the Adam's apple - yes, it is prominent now I've seen it, but it didn't jump out at me, I'd really just assume you were probably a lesbian, who preferred piercings to false eyelashes. Not a butch lesbian either, just one with a kind of alternative style. And therefore, female.

What's your aim? You really don't look out of the female range as it is, and once the shadow and the apple is gone, you could probably just go with what you're already doing. There's no point in loads of make up if the facial piercing stuff is your style preference instead. I honestly think it would have the opposite effect to the intention, but it's really up to you and how you envisage yourself.
