I just saw a video of Dr Thomas in Portland who explains his type of voice surgery (and the alternatives and additional procedures he could do). Since some people here asked why he is not mentioned so often, maybe this is interesting.
His main technique is the complete reconstruction "FemLar", which is very invasive. He also does laser thinning. He does not like to do CTA anymore and he has little experience with Wendler glottoplasty (or creation of an anterior webbing).

A nice complete and honest video. Sadly, his results don't sound any better than the minimally invasive voice fem surgeries.

Some of his FemLar results do sound good actually - not sure if I would say they sound better on average than with glottoplasty, they do sound better apparently than CTA-only, but so does glottoplasty. Of course he will have picked the good ones as well for the video - one thing that may be interesting is , if one could find out how much effort and training is required post OP with Dr Thomas. His technique claims to change resonance, something that has to be dealt with conventional speech training in all the other surgery techniques. But he is cauteous and says that he "believes it changes resonance". Its hard to really measure this parameter.
