Breast spacing concern

I've decide to have breast augmentation (BA).  My surgeon, Dr. Daniel Krochmal in Northbrook, IL, spent 2 hours with me in consultation to come up with an optimal choice of product and procedure to give me the look that I was after.  In short, I'd like breasts that are a large C-cup that don't look fake, which for me means not a lot of fullness on top.  I currently wear a 36B bra, so I'm not looking for a huge increase in volume.

He used the Vectra 3-D imaging simulator to help me decide on an appropriate implant.  He recommended going with a Mentor Cohesive III silicone, medium height, moderate profile 440 cc implant (gummy bear teardrop shape).  The implants will be place under the pectoral muscle through an incision in the breast crease. 

The Vectra produced these images (with bikini top) for this particular implant.  I was surprised at how much of a gap there is between the breasts.   Positioning the implants closer together causes the nipples to point more to the sides which is not a look that I like. 

Is this something that some of us trans women simply have to accept?  Is there any way to reduce the gap without producing side-pointing boobs?  I'm not really keen on going to a D or DD cup or to saline subglandular implants.  What is the experience of others here in regards to this issue?

I have 500 cc round silicone implants, which I think helps to narrow the gap, and my nipples point straight forward. My implants were a little too full looking on top for the first couple of years, but now they have settled into a more natural looking position.

Could you tell me your cup size and band measurement, kitten?  Were your implants placed above or below the muscle?

I have saline 600cc's on my 5'10 frame. I did not have a gap in my breast or my friends since we always wore bras  while on hrt so our boobs can grow forward and not east and west ...


EDIT: it's urban legend in the trans community to wear a bra in the beginning stages of hrt so ones breast can grow pretty.

A quick scan of the internet reveals that widely spaced breasts are a problem for many natal women, too.  It's a characteristic of one's anatomy that cannot easily be changed.  All breast implants must be centered under the nipple to look right.  A larger width implant will tend to lessen the gap between the breasts, but will also produce "side boobs" which may or may not be acceptable.  The width of an implant can also be limited by the amount of existing breast tissue (more is better), and where the pectoral muscles attach to the sternum (for submuscular placement).  It would appear that there is little that can be done to achieve cleavage when your breasts are naturally widely spaced.

Well it appear to me that chicks that did not invest in a bra usually have east and west tits ...cis or trans #next #boop #myOpinion

Bah! I have east-westies and I've always worn at least a sports bra. I agree with those who say it's all genetics.

I have East West too. I have a BA scheduled 9/16/16. I have to go over the muscle because my breast muscle is above my breasts. I will go with 450 or 550 silicone gummies. I have to decide Monday. I need to ask the doctor about contour or round and get her recommendation. She said previously that when I wear a bra I will have cleavage. The issue for me is that my breast plate protrudes outward more so than others.

So I was blessed by the hormone gods and they gave me a real nice pair of puppies but, one goes starboard and the other to the port.  It's great I have natural 36Ds but I can't get cleavage worth a damn unless I wear one of those ren fair breast smasher corsets. 
My point is, take advantage of the fact you are getting some professional work done and make sure the doc knows what you want.  I would guess that would be cleavage.  In your case with the pics, I humbly suggest you move up a cup or two.  Since we new girls have larger frames we also can host larger puppies and make it look natural. 
Also, if you get any body reshaping they can take from your upper back and forward armpit area to enhance how the girls seem to sit on your chest.
Also, it takes a while for hormones to do their bit.  SRS is a factor as well as progesterone.

My BA is scheduled for 9/15/16 and I'm going ahead with the 440cc medium height/modest projection anatomicals.  The implants measure 14 cm wide, 13.2 cm high, and 4.9 cm projection.  I wear a 36B bra, but my breasts barely fill the cups.  My doctor says it's as large as I can go given my existing breast tissue and still have my breasts look natural.  He also said, in the way of encouragement I suppose, that the Vectra 3-D tends to understate the size that will actually be achieved.   Let's hope.  He seemed pretty sure he could do better than the simulation.  I chose him based on reviews and the fact that he does all placements (subglandular, subfascial, subpectoral, and submuscular), all incision locations (breast crease, armpit, pariareolar, and through the belly button), and all implant types to meet his patients' needs.  So many surgeon have their pet approach and push it on all their patients which has to end up giving suboptimal results for some of those patients.  He made a point to lower my hopes for being able to have nice cleavage without a bra designed to squeeze the breasts together and upward.  Even that might be a stretch (pun intended   ).  It is what it is.
