Selecting Trans medical services in Orange County, CA & Medicare Plans

I am looking to change Medicare plans, and looking for an organization that provides good transgender services. I have heard about Kaiser Permante, but most of their facilities are in Northern CA.
Any comments or recommendations appreciated. the open enrollment clock is ticking.
