Advice Needed to FFS

Hello Ladies,

Need your advice on FFS. I have forehead reconstruction. The doctor Rhinoplasty as well. I do have a hump on the nose, but nose was the last thing I wanted to change. I am not sure if it is really much of a concern and whether it does make my face masculine.

By looking at the below pics/profile pic, what do you suggest? Would Rhinoplasty be a good idea?

Hey I'd recommend to do the forehead contouring on its own first. Upon research most FFS surgeons are not good with rhinoplasty, so I would avoid doing that with an ffs surgeon. Do you forehead surgery first with an FFS surgeon, then wait at least 4-6 months then do you rhinoplasty, with a plastic surgeon who specializes primary & revision rhinoplasty who only does noses. Rhinoplasty is a difficult area. So make sure you do your research and also it'll be great if they can do a before and after morph of your nose before surgery to show you what it'd look like. So I would definitely recommend to do them both as seperate procedures to maximise your feminine features.do the forhead first, then rhinoplasty second.

Hi Yashika,

My suggestion would be to do them together. You will be compromising your forehead feminization by not getting rhinoplasty. Also just curious r u considering Ffs in US or India.

Yps, in what way would you compromise forehead feminization if you do not combine it with rhinoplasty? I have the same impression as Venus in that rhinoplasty by FFS surgeons seems to be below average. I only like maybe 30% of the noses in post OP FFS pictures.

Based on the pics Yashika posted, it look like her brow bone is quite prominent and her nose bridge/radix is quite high too. In some other patients that had similar features and that opted only for forehead feminization without rhinoplasty, my observation was that the forehead setback was not optimal. This was because the surgeon didn't go for the maximum allowed forehead setback specially in the frontal sinus area because that would also require setback of the nose radix too to ensure smooth linear transition from nose into the forehead.

So in such cases, the surgeon will generally only bring it back till where the existing nose radix is, which means she still might end with a forehead overhang. Thus a masculine forehead becoming a androgynous one but not a feminine one.

Ok, so I guess it makes sense then to ask the FFS surgeon if they would do the forehead differently. Maybe suggest to them that one wants to keep the nose unchanged for now for whatever reasons - fear of surgery, money - but would consider doing it at a later stage and ask if that would be possible or not?

Venus, you are right, it is important that the surgeon be equipped well equipped and experienced for Rhinoplasty. This particular surgeon is quite famous for the rhinoplasty he performs, not just generally but also specifically for feminisation purposes. He has become a "go to person" for all surgeries. I did my Gender Correction and Voice Feminisation with him and am quite satisfied with the results.

Ypsf, that makes sense. Thank you so much! This helps a lot. As suggested by Anjaq, I will make it a point to discuss this with my surgeon. Unfortunately I am just 2 days away from the surgery date but I am sure he can make some arrangements. I will let him know that I do not want any compromise on the forehead setback. So in case he feels, it would  be best performed with a rhinoplasty, then will get both done. If he is says that it won't be an issue and he will not compromise on the forehead setback, then will probably get after 6 months if I feel am not able to pass. Funny thing though, I have a sharp nose, which am scared to lose. I have this feeling that he will make it feminine, sure,  but at the same time a little common and or perfect. I feel a little odd shape has its own attractiveness. hehe..call me crazy.

His name is Narendra Kaushik. He is located in New Delhi India. I had initially planned undergoing the gender correction surgery and FFs in Thailand by Dr. Chettawut, and found him quite skilled. But after visiting Dr. Narendra and checking  testimonials in Google, I decided to get it done by Dr. Narendra itself, as he was more experienced in combination of penile inversion + sigmoid colon.

Sigmois colon SRS seems to be quite invasive - cutting the colon is tough. I had my colon punctured during SRS and it was not great - I had to use a stoma for some weeks to have it heal... it was quite horrific. I guess if one get it, it has some advantages though, like not having to dilate, something which pains everyone else...

I agree Anjaq, it is quite invasive. I hope everything is alright. One of the downside is the scar left. Recovery wise I was lucky I guess. In 3 weeks was back to work. My digestive system also took some time to recover. But am glad about the dilation. After BA and FFS I found myself not dilating for days. Luckily that wasn't a problem

3 weeks is not that bad. I was out for 5 months because they had to reroute my intestines until the cut was healed. Is this not the case with sigmoid colon SRS? Its a technique not anymore performed in the western countries it seems.

Oh! 5 months is quite long! Am sorry for that. In fact I am just 4 months post-op. 1.5 months post-up I was able to resume exercise. The surgeon I went to is known for his sigmoid technique combined with penile inversion. He demonstrated his procedure before the surgery. Hi scraps the end portion of the colon (rectum). He said that this part has self healing properties, and would grow back to normal in no time. I did face some gastric problems initially (mild), and in no time was able to resume to normal diet (spicy food in my case). One of the downsides was discharge, which he said could last up to 6 months, though I don't see much of it. 2 months post-up things were pretty well.

Yeah they do work, depends on the scar as well. The scar left on the neck, has diminished substantially. will wait and see how the same goes for the one on the abdomen.

Scars can be something annoying. I have quite a few scars - breast surgery, the GRS complications, GRS scars themselves (hidden under the hair if I do not shave it too short), Broken leg scar,... some scars from some surgery near my genitals when I was 2...
It is quite a bit of scars and one time I was with a guy who was getting a bit suspicious why I have so many scars and why my clit looks a bit odd. I was scared to bits and could not enjoy sex with him, really - it was quite embarrassing and after we both went home to different cities, I refused to anser his calls. I did not want to come out as trans to him but apparently he was already onto it because of scars that actually had little to do with it.
