Having a foundation to help fund srs for Asians and having gone thru the surgery I thought I should give some unbiased views.....as much as possible, about what to expect and what to look for for your optimum results. I have read so many threads on this subject and I must admit some of my initial opinions were also fake news in that I expressed a biased view. So here we go:
The basic SRS surgery despite all the various surgeons marketing hype is basically the same. The aim to achieve aesthetics and orgasms reigns supreme.
Pre surgery
i) Laser and electrolysis of scrotal and penile skin pre surgery
I used to read that you needed laser or electrolisis on the penis and scrotal skin. I feel now that if you start electrolysis a year before surgery its actually not a bad idea. However, I am against laser for hair removal as its not permanent and makes the hair roots so small that they can be missed in the scrotal and penile skin scrape for hair roots resulting in hairs deep inside the vagina.
But electrolysis its not necessary on the scrotal or penile skin as the scraping process is very effective if diligently done, basically its the same process used by all surgeons. However, I do recommend that you remove the hair either by laser or electrolysis in the area between the anus and lower base of the penis. This area is not cut and there is a tendendicy for part of this skin to enter the vagina with dilation. Its may only go in 1/4 inch. This can also be done painlessly after surgery as the depth is so shallow. I know patients that have hair deep inside the vagina and this is caused by the technician being not diligent and laser work.
ii) Aesthetics
Different surgeons have different artistic intrepretation of what a labia minora and clitoris should be. Also remember that because you have to dilate some aesthetic changes need to be done a year after surgery when you can skip dilation for a couple of weeks while the corrective surgery stitching is secure.
The size of the inner labia is where you will see some major differences and each top surgeon has their preference. Be sure to state your preference, or find one that has your views of the ideal vagina. Remember vaginas come in all shape and sizes.
iii) Choosing surgeons
Practice makes perfect, its not rocket science surgery. All the top surgeons are capable, yet they all have some bad outcomes. But its rare. There are around 8 great surgeons that service overseas clients in Thailand. Results all similar but prices vary quite a bit.
iv) Surgery
Best if done in a hospital and not in a clinic. Surgeons vary on how long you stay in hospital and when you should walk. My personal view is 5 to 7 days in hospital, walk after 48 hours. I like that they remove the catheder before checking out of hospital. Some surgeons require being bedridden.
Many different problems occur but most of the problems are caused by swelling. They measure and cut very carefully but swelling distorts the measurements and may result in not an even labia, and necrosis. Most patients have little pain. My experience with my foundation and friends is around 20 pct have issues mentioned above. This does not include minor infections.
v) Post-op
Do not walk fast or excessivly for the first 3 weeks. This can tear the stitching at the vagina entrance and make dilation a nightmare. You have to dilate even if you have stitches torn. This can result in a persistant nagging pain that can be depressing. This nagging pain will go after 3 weeks.
vi) Dilation
Very important in the first few months. Its not going to be the sexual wonder many think. Its a daily chore and can be uncomfortable. Be persistant and it will be easier. Blood initally from micro tears when stretching the vagina is not a worry. After 4 months you should not have blood but it can vary.If you plan to have sex with a man, wait 6 months and be sure you are using the largest dilator. May look daunting at first but you will be able to use it. No rush, you can try the biggest dilator after 10 or 12 months.
I know some surgeons want you to dilate 3 times a day or more. My view is 2 time at 15 to 20 minutes will do. Be sure you keep the pressure so you dont lose depth. Also, use small circular movements with the dilator helps the depth and circumference.
vii) When can I sit and feel comfortable. Here it really varies but after 3 months you should be able to do your normal pre op activities.
I will talk about aesthetic changes and the options in another post. I just dont have enough information at this time.
