At the speed that my transition is going, it will be late 2020 or early 2021 before I get SRS, and I've been full time since January 2016. First I need help from SRS surgeons clarifying that it's okay to have a lower body lift to remove my excess skin prior to SRS (I lost 110lbs between 2011 and 2016), then the lower body lift itself, after an upper body lift. Then a BA, then FFS (which might be split into two surgeries separated by months). Then SRS, last due to planning for having to dilate and not wanting that to be interrupted by recovering from other surgeries. So it looks like I'll be walking around with chemically suppressed testes for another 3.5 years or so.
I so, so wish that I could have an orchiectomy, so that I wouldn't have to deal with them anymore. I switched from patches to injections in late April, and tried to reduce my Spiro has been decreased to revive my libido, which had died. A couple days ago, I had my T levels checked, and my T, which had been stable at 10ng/dl prior to switching to injections and reducing Spiro one increment, had now skyrocketed to 360ng/dl, causing me to fear remasculinization and immediately go back to an increased dose of Spiro. I'm just so done with having testicles, and wish I could have an orchie without worsening my SRS prospects. I hate this glacially slow surgical transition, mostly caused by issues of excess skin on my breast and genital area, and planning for how to deal with them before moving on to transition surgeries, which adds another step to the process and a lot of uncertainty.
I gave it some thought. Inguinal orchiectomy to reduce scar tissue, with testicular implants to ward off scrotal atrophy. I don't know if that would be enough, though. I heard also that there's penile atrophy post-orchie, but I don't know how that could be; if there was to be penile atrophy, wouldn't it already be there due to taking anti-androgens? I already combat that by making sure I get erections, to preserve my tissue for SRS. Wouldn't I just do the same post-orchi? Why would having 10ng/dl of Testosterone from having an orchie cause any more penile atrophy than having 10ng/dl of Testosterone from taking Spiro?
It's just such a pain in the ass, planning on having intact testicles until 5.5 years of HRT. I used to agonize over whether or not to keep the little bastards, until 1 year of HRT, but now I just want to get rid of the damn things.
