Passport Help

I am in the process of correcting my passport for my name change and gender change. My new birth certificate has F and my new name on it. I am planning on submitting on the DS 82 form which is for name change. Do I need to submit an original name change court order and original birth certificate, they are duplicate originals? Do I need to submit a copy of my GCS notarized document? Should I be submitting another form? I have my old passport that is 5 years old which I will be surrendering.

You need to us the DS-11 not the DS-83 if the gender is going to be changed. You should only need the new birth certificate (original) and definitely a court certified copy (with the seal) of the court order for the name change. Since your birth certificate is changed you shouldn't need the letter, but I would include it anyway.

Many times the person inspecting the forms will say to use a DS-83, but it is wrong. Bring a copy of the procedures from the state department's website from here: https://travel.state.gov/content/passports/en/passports/information/gender.html

I agree that you will need the DS-11 and not the DS-82 because there is a change of gender. The old passport is the proof of citizenship and must be included since it is still a legally valid passport ( it will be returned, voided). The name change (with the seal) is obviously necessary. Your drivers license will serve as proof of identity and residence, if its in your new name. I would suggest hurrying because the new Administration in Washington might change the rules.They are not trans friendly.
